ObjectivePain is one of the most common and serious complications after hemorrhoidectomy.Early postoperative analgesia is the key factor to recover the body function as soon as possible.At present,the main treatment of postoperative pain of mixed hemorrhoids is to use painkillers,such as non steroidal anti-inflammatory drugs?NSAIDs?,opioids and so on.Analgesics work fast,but the side effects can not be ignored.In addition,analgesics have ceiling effect[1].In this study,the anal pain after mixed hemorrhoids operation was used as the carrier to observe the clinical effect of electroacupuncture combined with ginger separated moxibustion.It is expected to find a more effective and safe treatment,and provide a broad direction for clinical work.MethodsPick 70 patients who had undergone the operation of mixed hemorrhoids?MMH?,from February to December 2019,in the anorectal Department of the second traditional Chinese Medicine Hospital of Sichuan Province and met the acceptance criteria of this trial were selected.According to the principle of control and randomization,the patients were divided into two groups according to the ratio of 1:1.Control group:take tramadol hydrochloride sustained-release tablets to relieve pain.Treatment group:Electroacupuncture at Changqiang,Erbai,Chengshan,Dachangshu and other acupoints combined with ginger separated moxibustion at Yaoshu.The frequency of electroacupuncture and ginger separated moxibustion was once a day,each time for 30 minutes.Main outcome measures:pain score:before treatment and on the 1st,2nd,3rd,4th and 5th days after treatment?peak value of pain after intervention?pain was scored by visual analogue score?VAS?.Secondary indicators:the time needed for pain to reach peak within 24 hours after operation,and the frequency of pain attack within 5 days after operation.Spss22.0 statistical software was used to establish database and analyze all data.Results?1?Baseline situation:the completion of the trial was good,the general data of the two groups before treatment were compared,P>0.05,the difference was not statistically significant,the two groups were comparable.?2?The pain score of the two groups within 5 days:comparison within the group:P1<0.01,the difference is statistically significant.Compared with the VAS score before treatment,each group can significantly reduce the VAS score every day after treatment.Comparison between groups:P2>0.05,there was no statistical difference between the two groups.On each day after treatment,the treatment group and the control group had the same effect on reducing VAS score.?3?Comparison of reducing the frequency of pain attack of subjects:comparison within the group:P1<0.05 on the 3rd and 5th day after operation in the control group compared with the previous day,the difference is statistically significant,which can reduce the frequency of pain attack;P1<0.05 on the 2nd,4th and 5th day after operation in the treatment group compared with the previous day,the difference is statistically significant,which can reduce the frequency of pain attack.Comparison between groups:P2>0.05 on the first day,the second day and the third day after operation,there is no statistical difference.The effect of the treatment group is equivalent to that of the control group,P2<0.05 on the fourth and the fifth day after operation,the difference is statistically significant,and the effect of the treatment group is better than that of the control group.?4?Comparison of the number of additional analgesics used in the two groups:the difference was statistically significant?P<0.05?.The number of additional analgesics used in the treatment group was less than that in the control group.?5?There was no statistical difference between the two groups.The two groups had the same effect on anal pain after mixed hemorrhoids operation.?6?Safety comparison:there was no statistical difference between the two groups?P>0.05?,but the incidence of adverse events in the treatment group was lower than that in the control group.Conclusion?1?The analgesic effect of electroacupuncture combined with ginger separated moxibustion on early anal pain after external stripping and internal ligation of mixed hemorrhoids is similar to that of tramadol hydrochloride sustained-release tablets.?2?Electroacupuncture combined with ginger separated moxibustion can not only reduce the pain intensity every day,reduce the number of pain attacks on the 2nd,4th and 5th day after operation,but also reduce the amount of additional painkillers.?3?Electroacupuncture combined with ginger separated moxibustion is a safe and effective method for early postoperative pain relief in patients with mixed hemorrhoids.
