Objective: to investigate the gender difference in the occurrence of pain in different time periods after external stripping and internal ligation, and to provide theoretical support for different analgesic intervention measures for different genders and different time periods after surgery. Methods: a total of 34 cases of male and 27 cases of female patients of mixed hemorrhoids underwent external stripping and internal ligation in the first ward of the anorectal department of the affiliated hospital of chengdu university of traditional Chinese medicine were collected.Postoperative pain questionnaire was distributed to postoperative patients. The basic information of the questionnaire included number, name, gender, age, ethnicity, internal hemorrhoid grading, number of incisions, operation completion time, etc. The self-filled information included pain number, relief time, and VAS visual score of pain.Patients in both groups received external peeling and internal ligation, conventional perioperative treatment, including preoperative routine preparation, postoperative intravenous drip of antibiotics, hemostatic drugs,peg-4000 powder to help soften the stool, and TCM intervention programs, including ear point burial-bean embedding, moxibustion, fumigation and sitz bath with addition and reduction of sophora flavescens, and oral administration of sanren decoction with dialectical TCM.Patients in the two groups were observed and recorded within 6h-24 h,48h and 72 h after surgery, and the data collected were statistically analyzed. Results: there was no significant difference between the two groups(P>0.05)in general conditions, which was comparable. There were no statistically significant differences in the number of pain, the proportion of daily pain, the average time of daily pain, the total time of daily pain, and the average VAS score of daily pain in the three days after surgery between the male and female groups by t test or rank sum test(P>0.05).Overall comparison within three days showed that there was a statistically significant difference between male patients and female patients in average subvasscore(P<0.05),but no statistically significant difference between male and female patients in total number and total time within three days(P>0.05).On the change of three days, the comparison of any two days within three days was statistically significant(P<0.05),the pain degree and time of patients in the two groups showed a general downward trend, but the number of pain was > on the second day,> on the first day and > on the third day. Conclusions: gender difference had an effect on postoperative pain in patients with mixed hemorrhoids, but the effect was small.Male and female patients had the same pain distribution pattern and pain degree. On average, the women's scores were more concentrated and the men's scores more discrete for each of the three days after surgery. In terms of time, the overall trend of postoperative pain in the three days after surgery was downward, and the differences were statistically significant. In the second day after surgery, the pain frequency is more and the duration is longer. In the clinical development of analgesic plan after external stripping and internal ligation, gender factors have a small impact, but the score dispersion degree of male patients for pain degree is greater, for the male patients with lower pain threshold, analgesic measures should be actively used.
