Objective To understand the current situation of the implementation of the family doctor policy in Sichuan Province,to dig into the existing problems in the implementation of the family doctor policy,and to explore the reasons,and to put forward countermeasures and suggestions for optimizing the implementation effect of the family doctor policy.Methods Firstly,literature analysis was used to sort out relevant policy documents and journal literature to understand the current situation and development trend of family doctor policy implementation;Secondly,by means of questionnaire survey,data such as the signing status of resident family doctors,the factors affecting the signing,the willingness of unsigned residents to sign the contract,the willingness of signed residents to renew the contract,the working status of family doctors,the service mode of signing the contract,the cognition of policy,and the job satisfaction were collected;Third,through statistical description,chi-square test,non-parametric test,regression analysis and other statistical methods for statistical analysis of the data,and problems in the implementation of the policy are found;Finally,the comprehensive policy execution model is used to analyze the causes of family doctor policy implementation problems from three aspects: the characteristics of policy problems,the factors of the policy itself,and factors outside the policy.Results The survey results show that the contract coverage rate of family doctors in Sichuan Province is 27.6%;age,health status,area,access to medical treatment,knowledge of policies are factors that affect residents' contracting;residents who sign the contract are not willing to renew,and the service satisfaction needs to be improved;the willingness of unsigned residents to sign a contract is affected by both internal and external factors.The majority of the family doctor team in terms of membership composition includes doctors,nurses,and public health personnel;the medical staff has a good self-evaluation on work competency,which is positively related to the training times;the general practitioner,general practitioners,lower professional titles,and younger medical personnel training times are more;the medical staff has a lower job satisfaction,and the satisfaction of "matching income and workload" is the lowest.The main problems in the implementation of the family doctor policy in Sichuan are the low acceptance of family doctor policies,low residents 'awareness of the policy,weak residents' willingness to renew the contract,and low family doctor job satisfaction.Conclusions The reasons for the problems in the implementation of the family doctor policy in Sichuan Province are mainly in the following aspects: first,the characteristics of the policy issues are complex and difficult to implement,the number of policy target groups is large,and the behavior needs to be adjusted to a large degree;Second,the factors of the policy itself include the correctness of the policy itself but the lack of rationality of the goal,the lack of clarity of the policy content and the lack of policy resources.Third,the factors other than the policy include residents' weak sense of identity to the policy,the quality of the executive staff to be improved,the unsound policy supervision and evaluation mechanism,and the constraint of the policy environment.According to the research,it is suggested that the multi-department linkage mechanism should be established;expand publicity channels and deepen the connotation construction of contracting services;improve policy rationality and set goals according to local conditions;improve and implement related supporting policies;increase policy resource support;improve residents' willingness to sign contracts;attract high-quality talents to join the family doctor team;improve the policy supervision and evaluation mechanism,etc.
