Research On The Implementation Status And Problems Of Medical Liability Insurance Policy Based On Horne Mitt Model

In recent years,the number of medical disputes has been increasing year by year,and violent injuries to doctors have emerged one after another,which seriously interferes with the healthy development of medical and health undertakings.It is urgent to strengthen the construction of medical risk sharing system.S city of Shandong Province has implemented the medical liability insurance policy since 2016.In order to deeply explore the operation status and problems existing in the implementation of medical liability insurance policy in S City,and improve the implementation effect of medical liability insurance policy,this study takes s city as a case study,takes Horne mitt model as the theoretical basis,and adopts the research method of combining qualitative and quantitative analysis.Literature research,interview and questionnaire were used in this study.Firstly,the research status of medical liability insurance and policy implementation at home and abroad is clarified through systematic review of relevant literatures at home and abroad.This paper summarizes,compares and evaluates the medical liability insurance models at home and abroad,and looks for the problems existing in the domestic medical liability insurance system and its reference significance to this study.Secondly,select the heads of representative medical institutions,insurance companies and relevant government departments for unstructured interviews as the basis of qualitative analysis.At the same time,combined with the six dimensions of horne mitt model,carry out a questionnaire survey on the implementation status and effect evaluation of medical liability insurance policy in s city for medical personnel and patients at all levels of medical institutions.According to the interview and questionnaire survey results,comprehensively analyze and evaluate the implementation status of medical liability insurance policy in s city from the six dimensions of horne mitt model.Then,through spss24 the qualitative analysis and quantitative analysis of the factors affecting the implementation of the medical insurance policy in s city were carried out,combined with the qualitative analysis and quantitative analysis of the factors affecting the reliability and effectiveness of the medical insurance policy in s city.Finally,based on the above analysis results,an optimization strategy is proposed to further improve the policy implementation effect in S City.Through research and analysis,it is concluded that:(1)policy objectives and standards,policy resources,policy implementation methods,system environment and the value orientation of target groups are significantly positively correlated with the implementation effect of medical liability insurance policy,and the characteristics of policy implementation institutions are not significantly correlated with the implementation effect of medical liability insurance policy.(2)Current S city medical liability insurance policy implementation effect from the target,there is still a gap insufficient understanding of the policy,medical staff and patients’ satisfaction is not high,unable to fully meet the demand of the target group,the way of carrying enforcer’s value orientation.The main reason is not enough refining,policy propaganda policy objectives and standards do not reach the designated position,inadequate financial resources and information resources allocation,poor environmental policy system.(3)combined with horne mitt model,this paper puts forward the optimization strategies to promote the implementation of medical liability insurance policy and system construction in s city,including setting scientific and differentiated premium rates,establishing a multi insurance sharing medical risk mechanism,increasing financial subsidies,improving the construction of information system,strengthening the communication and coordination between policy executors,legislative enforcement and strengthening publicity.
