Objective: To observe the clinical efficacy and safety of the traditional Chinese medicine Dahuanggancao Tang retaining enema in the treatment of acute radiation proctitis(Redushangluo Syndromes)in low and middle rectal cancer.Research objects and methods: From February 2019 to January 2020,62 patients who met the requirements for inclusion in this study at the Oncology and Anorectal Clinic and Inpatient Department of Sichuan Provincial Hospital of Traditional Chinese Medicine and the Oncology Outpatient and Inpatient Department of Sichuan Provincial People's Hospital were collected.Patients with intermediate and low rectal cancer diagnosed as stage ? and ?(32 cases in the test group and 30 in the control group).Both groups were treated with neoadjuvant concurrent chemoradiotherapy,using intensity-modulated radiotherapy(IMRT).The radiation dose was basically the same.During the chemotherapy,5-FU(fluorouracil)was used.Medication scheme: the control group retains the enema fluid(dexamethasone sodium phosphate injection 5mg +gentamicin sulfate injection 80,000U+montmorillonite powder 3g+0.9% physiological saline 100 ml mixed solution(qd)),the test group retains the enema fluid(Traditional Chinese medicine " Dahuanggancao Tang"(raw rhubarb 30 g,raw licorice 10 g,qd)),all enemas were started two weeks after the completion of radiotherapy on the 15 th day,and all were administered until the end of radiotherapy.Observe and record the time of first occurrence of ARP,intestinal response classification,routine stool occult blood test,traditional Chinese medicine syndrome score,KPS score(quality of life score),safety indicators and adverse drug reactions in two groups of patients.Data analysis was performed using statistical software.in conclusion.Results: A total of 60 patients completed the study(30 patients in the control group and 2 patients in the test group were removed from the study group).(1)Before treatment,the general conditions of the two groups of patients were: gender,age,pathological type,lesion location,tumor staging,RTOG classification of acute radiation injury,fecal occult blood test,TCM symptom score,and KPS score(quality of life score)were at a comparable level(P > 0.05).(2)The time of the first appearance of ARP in the two groups was statistically significant in the effects of acute radiation injury,conventional occult blood test in stool,efficacy of traditional Chinese medicine syndrome,KPS score difference(effect),and safety evaluation indicators after treatment(P < 0.05).Conclusion: "Dahuang Gancao Tang" retention enema can effectively prevent and treat ARP in middle and lower rectal cancer,effectively increase the body's tolerance to radiation,delay the first appearance of ARP,reduce the degree of acute rectal radiation damage and the grade of intestinal reactions,and make TCM symptoms It has been improved,and the quality of life has been improved after the pain has been reduced,so that the patient can spend the radiation therapy period more safely,and the clinical use is safe and effective.
