Study On Correlation Between TCM Syndrome Types And Clinical Indexes In Patients With Type 2 Diabetes Mellitus With Coronary Heart Disease

Objective: based on the TCM Syndrome Distribution of Patients with type 2 diabetes Mellitus(T2DM)and coronary heart disease(CHD),to study the correlation between TCM Syndrome types and clinical objective indexes,in order to supply objective basis for TCM syndrome differentiation,to standardize the treatment of Type 2 diabetes with coronary heart disease by TCM Syndrome differentiation.Methods: 1)from December 2018 to December 2019,222 inpatients from the cardiovascular department in the Sichuan Provincial Hospital of Traditional Chinese Medicine were selected as the study object。2)the qualified cases were selected according to the inclusion and exclusion criteria of type 2 diabetes complicated with coronary heart disease,and the syndrome differentiation of TCM was classified in the light of reference to Chinese medical standard for diagnosis and treatment of diabetes complicated with coronary heart disease by three associate directors or chief physicians。3)to collect the clinical data of 222 patients including the four diagnosis information of TCM,age,sex,BMI,course of Type 2 diabetes,course of Coronary Heart Disease,index of Glucose Metabolism(FPG,2h PG,Hb A1c),index of Lipid Metabolism(TC,TG,LDL-C,HDL-C),Angiotensinogen II,homocysteine,etc.4)all data are entered into excel system and analyzed by SPSS25.0 software.Results: 1.Among the 222 patients,105 were male,117 were female,the mean age(57.64 ± 10.95)years old,63(28.4%)were deficiency of Deficiency of both Qi and Yin;,52(23.4%)were Syndrome of stagnation of heart vessels,47(21.2%)were stagnation of phlegmturbidity,28(12.6%)were deficiency of both Yin and Yang,there were 20 cases(9.0%)of heart-kidney Yang deficiency syndrome,12 cases(5.4%)of water-qi ling-xin syndrome。 3.Sex: 6 GROUPS OF TCM Syndrome sex distribution: qi-yin deficiency syndrome male 20 cases,female 43 cases,Phlegm Block Syndrome male 27 cases,female 20 cases,heart-pulse Stasis Syndrome male 28 cases,female 24 cases,yin-yang Deficiency Syndrome male 11 cases,female 17 heart-kidney Yang Deficiency Syndrome male 12 cases,female 8 cases,there are Significant differences in gender composition between the six groups(p < 0.05)(211.551,p < 0.041).4.Age: The difference of Syndrome types in 6 groups was statistically significant.The age of patients with yin-yang Deficiency Syndrome,heart-kidney-yang Deficiency Syndrome and Water-qi Ling-xin Syndrome was significantly higher than that of patients with phlegm-turbid Stagnation Syndrome,qi-yin deficiency syndrome and heart-vessel Stasis Syndrome,there was significant difference between the two groups(p < 0.01).The course of Type 2 diabetes and the course of coronary heart disease: there was a significant difference between 6 groups of TCM Syndrome types.The course of DM of deficiency of Yin and Yang,deficiency of water and Qi Lingxin,deficiency of heart and Kidney Yangwas greater than that of deficiency of Qi and Yin,stagnation of phlegm and stagnation of heart vessels,the difference was statistically significant(p < 0.05).5.Glycometabolism Index: 1)there was significant difference between 6 groups of TCM syndrome(p < 0.01),Hb A1 c of qi-yin Deficiency Syndrome was significantly higher than that of other tcm syndrome groups,and the disparity was statistically significant(p < 0.05).2)two-hour Postprandial Blood Glucose(2HPG): There was significant difference between the six groups(p < 0.01).The 2h PG of qi-yin Deficiency Syndrome was significantly higher than that of other TCM syndromes(p < 0.05).6.Lipid Metabolism Index: 1)Serum Total Cholesterol(TC): there was significant difference in tcm syndrome types between the six groups.TC of phlegm-turbid Block Syndrome and heart-vessel Stasis Syndrome was significantly higher than that of qi-yin Deficiency Syndrome,Water-qi Ling-xin Syndrome and heart-kidney Yang Deficiency Syndrome(p < 0.05).TC of qi-yin Deficiency Syndrome was significantly higher than that of Water-qi Ling-xin Syndrome(p < 0.05)。2)LDL-C: There was significant difference between 6 groups in TCM Syndrome types.LDL-C of qi-yin Deficiency Group was significantly higher than that of other groups(p < 0.05).7.Homocysteine: There were significant differences between the six groups in TCM syndrome types.The level of qi-yin Deficiency Syndrome was significantly higher than that of heart-vessel Stasis Syndrome,yin-yang Deficiency Syndrome,heart-kidney-yang Deficiency Syndrome and Water-qi Ling-xin Syndrome,there was significant disparity(p < 0.05)between the two groups,and the level of homocysteine in the group of phlegm-turbidity Syndrome was significantly higher than that in the other five groups(p < 0.05).8.Angiotensinogen II(Angii): There was a significant difference between 6 groups in Tcm Syndrome types,the phlegm-turbidity Syndrome group was higher than the other 5 groups in tcm syndrome types,the difference was statistically significant(p < 0.05),qi-yin Deficiency Syndrome are more than the Yin-yang Deficiency Syndrome,the difference was statistically significant(p < 0.01),and the disparity was statistically significant(p < 0.05).9.Body Mass Index(BMI),Fasting Blood Glucose(FPG),high-density Lipoprotein(HDL-C)and triglyceride: There was no significant difference between the six groups(p > 0.05).Conclusion: 1.qi-yin Deficiency Syndrome is the most common type of TCM Syndrome in type 2 diabetes mellitus complicated with coronary heart disease,followed by heart-vessel Stasis Syndrome and phlegm-turbidity stagnation syndrome,the older,the course of diabetes and coronary heart disease were more yin-yang Deficiency Syndrome,heart-kidney-yang Deficiency Syndrome,Water-qi Ling-xin Syndrome.2.Angiotensinogen II,homocysteine,Glycated Hemoglobin,2h Postprandial Blood Glucose,cholesterol,and Low-density Lipoprotein are approaching connected TCM Syndrome differentiation of type 2 diabetes with coronary heart disease,it can be used as an objective reference index for TCM Syndrome differentiation of Type 2 diabetes Mellitus with coronary heart disease...
