Objective:Objective To investigate the effect of Buyang Huanwu Decoction on the rapid recovery of qi deficiency and blood stasis type elderly patients with intertrochanteric fractures.Methods:A retrospective study of 80 patients with Intertrochanteric fractures who had been admitted to the Department of Orthopaedics of the Affiliated Hospital of Chengdu University of TCM from January 2017 to January 2019 and had received closed reduction and internal fixation(proximal femoral nail anti-rotation(PFNA)).40 cases of Chinese medicine were added to the basis of rapid rehabilitation,40 cases were not Chinese medicine;Collected before surgery,day 1 and 2 weeks the preprothrombin time(PT),activated partial prothrombin time(APTT),fibrinogen(FIB),D-dimer(D-D),hemoglobin(Hb),TCM syndrome scores and safety indicators,and Observed hip function scores before surgery,1 and 2 weeks after surgery,then analysis results.Result:1?There were no significant differences in age,gender,Evens fracture classification,CCI(comorbidity index score),Harris score,and Hb,PT,APTT,FIB,DD,and TCM symptom scores before surgery in the two groups(P>0.05);2?Hb:2 weeks after operation,the hemoglobin level of the Chinese medicine group(110.18±8.34)was significantly higher than that of the non-Chinese medicine group(100.13±13.78),and there was a statistical difference(P<0.05);3?Two weeks after operation,the PT and APTT(14.1±0.81,37.80±2.49)of the Chinese medicine group were higher than those of the non-Chinese medicine group(12.27±1.06,36.67±2.39)and P<0.05;4?After 2 weeks,FIB and D-D(4.06±0.94,3.21±0.89)of the Chinese medicine group were lower than those of the non-Chinese medicine group(4.61±1.04,4.17±2.54)and P<0.05;5?Harris score:The Harris score(40.7±5.99,57.48±8.76)of the Chinese medicine group was better than that of the non-Chinese medicine group(36.88±4.85,53.4±7.47)at 1 week and 2 weeks after operation,and P<0.05;6?TCM symptom score:2 weeks after surgery,the TCM symptom score of the Chinese medicine group(7.10±4.49)was significantly lower than that of the non-Chinese medicine group(10.08±4.03)and P<0.05;7?Symptom improvement rate:2 weeks after surgery,there was no statistical difference in the overall symptom improvement rate between the two groups(P>0.05);but the effective rate in the Chinese medicine group was 47.5%significantly higher than that in the non-Chinese medicine group by 20%,and P<0.05;8?Complications:The total complication rate was not statistically different between the two groups(P>0.05),but the lung infection rate in the Chinese medicine group was 17.5%significantly lower than the non-Chinese medicine group at 37.5%,and P<0.05;9?Safety evaluation:No abnormalities in vital signs,liver and kidney function,electrolytes,and electrocardiogram were found during the administration of the two groups,and no adverse reactions occurred.Conclusion:Buyang Huanwu Decoction can significantly improve the symptoms of Qi deficiency and blood stasis and PT,APTT,FIB,and D-D levels after intertrochanteric fracture without increasing the risk of bleeding.It can also reduce the incidence of postoperative pulmonary infection,promote postoperative hemoglobin and early Hip function restored.In summary,Buyang Huanwu Decoction has a positive effect on the rapid recovery of elderly patients with intertrochanteric fractures and is safe and effective.
