| Objectives To explore the correlation between post-stroke depression(PSD)and atherogenic index of plasma(AIP),apolipoprotein B/apolipoprotein AI(apoB/apoAI)and carotid artery atherosclerosis.Methods A total of 178 patients with ischemic stroke admitted to the affiliated hospital of north China university of science and technology from May 2018 to August 2019 were selected as the study subjects.The degree of depression was assessed by Hamilton Depression scale(HAMD)and divided into groups,the HAMD score <8 was defined as non post-stroke depression(NPSD)group,and the HAMD score ?8 was divided into PSD group,and in line with the consensus of Chinese experts on the clinical practice of post-stroke depression,among them,HAMD score is 8~20 as mild depression group,HAMD score 21~35 as moderate depression group,and HAMD score >35 as severe depression group;the biochemical indexes such as high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglyceride(TG),apoB and apoAI were measured,and AIP and apoB/apoAI were calculated,and the carotid intima-media thickness(CIMT)is used as an imaging index to reflect the condition of carotid artery atherosclerosis;the degree of neurological impairment was evaluated by the National Institutes of Health Stroke Scale,the Activities of Daily Living Scale(ADL)was used to evaluate the activities of daily living.Results 1 Compared with the basic clinical data of PSD group and NPSD group,there was significant difference in stroke times(onset/recurrence)(P<0.05),there was no significant difference in sex,age,education level,marital status,body mass index,history of hypertension,family history of mental illness,smoking and drinking history(P>0.05).2 In the PSD group,there were 39 patients with mild depression(47.6%)accounted for a large proportion,28 patients with moderate depression(34.1%)and 15 patients with severe depression(18.3%).3 It was found that the level of LDL-C in PSD group was higher than that in NPSD group,the level of HDL-C in PSD group was lower than that in NPSD group,and the difference was statistically significant(P<0.05);comparison of TC and TG levels between the PSD group and the NPSD group showed no statistical significance(P>0.05).4 NIHSS score of the PSD group was higher than that of the NPSD group,and the difference was statistically significant(P<0.05);ADL score of PSD group was lower than that of NPSD group,and the difference was statistically significant(P<0.05).5 AIP,apoB/apoAI and CIMT levels of patients in the PSD group were higher than those in the NPSD group,with statistically significant differences(P<0.05).6 In the PSD group,there were significant differences in the levels of AIP,apoB/apoAI and CIMT among mild,moderate and severe depression groups(P<0.05),the more serious the degree of depression in PSD patients was,the higher the levels of AIP,apoB/apoAI and CIMT were.7 The HAMD score of PSD patients was negatively correlated with ADL score and HDL-C level(P<0.01),positively correlated with NIHSS score,AIP,apoB/apoAI and CIMT level(P<0.01),and positively correlated with LDL-C level(P<0.05);the correlation between AIP,apoB/apoAI and CIMT and HAMD score in PSD patients was higher than that between LDL-C and HAMD score;there was a positive correlation between the level of apoB/apoAI and CIMT(P<0.05).8 The results of Logistic regression analysis show that,HDL-C is a protective factor for PSD;LDL-C,AIP,apoB/apoAI,and CIMT are the risk factors of PSD.Conclusions The levels of AIP,apoB/apoAI,CIMT are the risk factors of PSD,and are related to the severity of depression in patients with PSD,the more serious the degree of depression in patients with PSD,the higher the levels of AIP,apoB/apoAI,CIMT,in the process of clinical examination of stroke patients,we should pay attention to the level of three indexes,and some psychological intervention can be carried out in the early stage to avoid the occurrence of PSD.Figure 0;Table 8;Reference 192... |
