Objective To evaluate whether the value of Atherogenic index of plasma(AIP)in predicting the severity of atherosclerotic lesions and plaque stability is better than other traditional blood lipid Parameters.Objective to investigate the value of AIP in predicting the severity of atherosclerosis in normal blood lipid indexes people.Methods Our survey Based on 2015 stroke prevention and screening program of National health development planning commission.we select the more-than-40-year old(including 40 years)residents(Residence time ?half an year)in Tanyang district of changha by cluster sampling method.The Basic date(e.g.Name,Sex,age and medication history)were collected By filling out the questionnaire From November 2015 to January 2016.Study subjects were the the study participants excluded who were taking lipid-modulating drugs in Nearly 3 months and those with a serious lack of information.The clinical data of study subjects were collected including Carotid artery ultrasonography,Total cholesterol(TC),Triglyceride(TG),High density lipoprotein cholesterol(HDL-C),Low density lipoprotein cholesterol(LDL-C))and AIP calculated by the results of TG and HDL-C.The measurement data were expressed by mean±standard deviation(SD),categorical variables were expressed as frequency(percentage).All the study subjects were divided into non-plaque group,plaque group and stenosis group according to the results of carotid artery ultrasonography.According to the nature of the plaque,all the study subjects with Atherosclerotic plaque were divided into stable plaque group and unstable plaque group.The variability of AIP value and plasma TC,TG,HDL-C,LDL-C concentration were analyzed by two-sample t-test.the correlation ship of AIP value and plasma TC,TG,HDL-C,LDL-C concentration was analyzed with Pearson correlation.Then,according to?Guidelines for prevention and treatment of dyslipidemia in Chinese adults2016?,the study subjects with normal blood lipid indexes were screened.The study subjects with normal blood lipid indexes were also divided into non-plaque group,plaque group and stenosis group according to the results of carotid artery ultrasonography.The variability of AIP value among The three groups of study participants with normal blood lipid indexes were analyzed by two-sample t-test.the correlation ship of AIP value among these three groups was analyzed with Pearson correlation.Result 1.Based on the census data of Human Provincial Bureau of statistics in 2015,according to the household registration data,in accordance with the age and residence requirements of the survey,A total of 1398 people were surveyed,1264 study participants have complete data in the research. 378 study participants were excluded because of taking lipid-modulating drugs in Nearly 3 months.866 study subjects were included in this study.according to ? Guidelines for prevention and treatment of dyslipidemia in Chinese adults 2016?,a total of 271 study subjects with normal blood lipid indexes were screened in all the study subjects(n=866).2.According to the results of carotid artery ultrasonography,487 study subjects were included in the non-plaque group,233 study subjects were included in the plaque group,146 study subjects were included in the stenosis group among all the study subjects(n=866).The prevalence of the three groups was: non-plaque group 56.23%,plaque group 26.91%,and stenosis group 16.86%.214 study subjects were included in the stable plaque group,165 study subjects were included in the unstable plaque group among the all study subjects with Atherosclerotic plaque(n=379).197 study subjects were included in the non-plaque group,66 study subjects were included in the plaque group,8 study subjects were included in the stenosis group among the study subjects with normal blood lipid indexes(n=271).The non-plaque group,plaque group,and stenosis group consisted 72.70%,24.35%,and2.95% of all the study subjects with normal blood lipid indexes.3.In all the study subjects(n=866),the AIP value of non-plaque group,plaque group,and stenosis group were 0.14±0.15,0.32±0.14,0.36±0.15.The plasma concentration of TC of non-plaque group,plaque group,and stenosis group were 5.03±0.73mmol/l,5.78±0.80mmol/l,5.70±0.77mmol/l.The plasma concentration of TG of non-plaque group,plaque group,and stenosis group were 1.57(1.29-2.14)mmol/l,2.24(1.67-3.12)mmol/l,2.28 (1.71-3.12)mmol/l.The plasma concentration of LDL-C of non-plaque group,plaque group,and stenosis group were 2.76±0.55mmol/l,4.01±0.86mmol/l,4.06±0.82 mmol/l.The plasma concentration of HDL-C of non-plaque group,plaque group,and stenosis group were 1.24±0.16mmol/l,1.08±0.76mmol/l,1.03±0.11mmol/l.The study Subjects in the plaque group and the stenosis group had significantly higher AIP value,TC,TG,LDL-C,compared with the study subjects in the non-plaque group,all differences were statistically significant(P < 0.05).The study Subjects in the plaque group and the stenosis group had significantly lower HDL-C,compared with the study subjects in the non-plaque group,the difference was statistically significant(P < 0.05).AIP value was significantly higher in the stenosis group compared to the plaque group(P=0.03).There were no significant differences in TC,TG,LDL-C,HDL-C of the stenosis group and the plaque group(P > 0.05).The AIP value,TC,TG and LDL-C were correlated positively with the severity of atherosclerotic lesions(r=0.58,p=0.000,r=0.32,p=0.01,r=0.27,p=0.04,r=0.008,p=0.007).the HDL-C correlated negatively with the severity of atherosclerAIP value with the severity of atherosclerotic lesions was was significantly higher than TC,TG and HDL-C with the severity of atherosclerotic lesions.4.In all the study subjects with Atherosclerotic plaque(n=379),the AIP value of stable plaque group and unstable plaque group were 0.31±0.14,0.36±0.14.The plasma concentration of TC of stable plaque group,and unstable plaque group were 5.82±0.73mmol/l,5.65±0.77mmol/l.The plasma concentration of TG of stable plaque group and unstable group were 2.24(1.66-3.11)mmol/l,2.27(1.69-3.12)mmol/l.The plasma concentration of LDL-C of stable plaque group and unstable group were 4.04±0.75mmol/l,4.02±0.69mmol/l.The plasma concentration of HDL-C ofstable plaque group and unstable group were 1.07±0.73mmol/l,1.03±0.15mmol/l.The study Subjects in the stable plaque group had significantly higher AIP value compared with the study subjects in the unstable plaque group,the differences were statistically significant(P<0.05).There were no significant differences in TG,TC,LDL-C,HDL-C of the two groups(P>0.05).The AIP valuewere correlated positively with the stable of plaque(r=0.29,p=0.01).5.In the study subjects with normal blood lipid indexes(n=271),the AIP value of non-plaque group,plaque group,and stenosis group were0.07±0.02,0.14±0.07,0.16±0.09.The study Subjects with normal blood lipid indexes in the plaque group and the stenosis group had significantly higher AIP value,compared with the study subjects with normal blood lipid indexes in in the non-plaque group,the difference was statistically significant(P<0.05).There were no significant differences in AIP value of the stenosis group and the plaque group(P >0.05).The AIP value also were correlated positively with the severity of atherosclerotic lesions in the study subjects with normal blood lipid indexes(r=0.37,p=0.01).Conclusion 1.The results suggest that AIP value,TC,TG,LDL-C increased and HDL-C decreased are important risk factors for atherosclerosis.Our data firstly suggest that the value of AIP in predicting the severity of atherosclerotic lesions is better than TC,TG and HDL-C.2.AIP value increased are important risk factors for the plaque stable.Our data suggest that the value of AIP in predicting the stable of atherosclerotic plaque is better than TC,TG,LDL-C and HDL-C.3.AIP also has the value of predicting the severity of atherosclerotic lesions in normal blood lipid indexes people.
