Objectives To investigate the incidence of post-stroke depression(PSD)in acute phase and its influencing factors,to analyze the relationship between PSD and carotid artery atherosclerosis.Methods A total of 358 cases of ischemic stroke were enrolled from department of neurology in Affiliated Hospital of North China University Science of Technology from November 2015 to October 2016.According to the score of Hamilton depression(HAMD),those who got the aggregate score <8 points were called the non-PSD group,while the others were called the PSD group.The incidence of PSD was observed.The demographic factors,previous history,stroke lesions,serological homocysteine(Hcy)and highsensitivity C-reactive protein(hs-CRP),carotid intima-media thickness(CIMT)and carotid atherosclerotic plaque were compared between the two groups,and to explore the risk factors of PSD.Results 1 The incidence of PSD acute phase was 31.3%,mostly mild to moderate depression patients.2 There were statistically significant differences in age between the two groups(P<0.05).The mean age of PSD patients was higher than that of non-PSD group.3 There was no significant difference between the two groups in hypertension,diabetes mellitus,coronary heart disease,smoking and drinking history(P>0.05).4 The proportion of patients with multiple stroke and multiple lesions in PSD group was significantly higher than that in non-PSD group(P<0.05);There were significant differences in the lesions locations of PSD patients with non-PSD patients(P<0.05);The ratio of frontal and basal ganglia in PSD group was higher than that in non-PSD group,the difference was statistically significant(P<0.05),but there was no significant difference in temporal lobe,parietal lobe,occipital lobe,cerebellum and brainstem lesions(P>0.05).5 The mean NHISS and BI scores in the PSD group were higher than those in the non-PSD group(P<0.05),and the higher the degree of depression in the PSD patients,the higher the NHISS and BI scores.6 The levels of serum Hcy and hs-CRP in PSD group were significantly higher than those in non-PSD group(P<0.05),and the higher the degree of depression and the higher levels of serum Hcy and hs-CRP in PSD patients.7 The detection rate of CIMT thickening in the PSD group(95.5%)was higher than that in the non-PSD group(74.4%)(P<0.05),and the thickness of CIMT in PSD group(1.73±0.66mm)was higher than that in non-PSD group(1.34±0.42 mm)(P<0.05);The rate of plaque in PSD group(86.6%)was higher than that in non-PSD group(71.5%)(P<0.05),and the patch area(24.31 ± 27.44 mm2)in PSD group was higher than that in non-PSD group(14.68 ± 18.49 mm2)(P<0.05);There was no significant difference in patch location and plaque between the two groups(P>0.05).8 Logistic regression analysis showed that the number of strokes,basal ganglia lesions,NHISS score,serum hs-CRP and CIMT were independent risk factors for PSD.Conclusions 1 PSD is a common complication of stroke patients,with a high incidence,of which the incidence of PSD patients mainly mild to moderate depression.2 The number of strokes,basal ganglia lesions,NHISS score,serum hs-CRP and CIMT were independent risk factors for PSD,and clinical screening could refer to the indicators.
