Objective To explore the clinical application value of the simple assessment scale for progressive ischemic stroke,so as to provide objective basis for the prevention and treatment of progressive ischemic stroke and facilitate the wide application of the scale.Methods Collected patients with acute cerebral infarction who were hospitalized from Affiliated Hospital of North China University of Technology from October 2016 to December 2017.According to the diagnostic criteria,inclusion criteria and exclusion criteria,809 patients were selected.All the patients were scored by NIHSS Score when they admited to hospital and their condition changed.All patients were divided into progressive ischemic stroke(PIS)group and non progressive ischemic stroke(NPIS)group according to whether the NIHSS score increased by 2 points or more within 6 hours to 7 days.Progressive group includes 198 cases and non-progressive group includes 611 cases.The clinical information and related medical history of patients were collected at admission,and the patients were scored with simple assessment scale.The measurement data were analyzed by t-test,and the enumeration data were analyzed by ?~2-test.The correlation between simple assessment scale and progressive ischemic stroke was explored by analyzing the scores of the two groups.By drawing the working characteristic curve(ROC)of the subjects scored by the simple scale,the best boundary value was obtained.According to the actual clinical outcome,the consistency test(kappa value)and paired chi-square test were carried out to further explore the clinical application value of the simple assessment scale for progressive ischemic stroke.Finally,296 eligible patients with acute ischemic stroke who were hospitalized from January 2018 to August 2018 were collected.The validation analysis of the clinical application value of the simple assessment scale for progressive ischemic stroke were carried out.Results 1 There was significant difference in the scores of simple assessment scale between progressive group and non-progressive group(P<0.05),and the value of the progressive group was higher than that of the non-progressive group.2 Using the mean of the simple assessment scale as the bounded point,all patients was divided into high assessment group and low assessment group,and the chi-square test was carried out.The results showed that there was a significant difference in the composition ratio of high simple scale between the progressive group and the non progressive group(P<0.05).3 The results of multiple factor logistic regression analysis showed that the score of the simple assessment scale was related to the progressive ischemic stroke.And the simple assessment scale score increased,the risk of progressive ischemic stroke increased.The difference is statistically significant(P<0.05).4 At evaluation stage of clinical application value of simple assessment scale for progressive ischemic stroke,the area under ROC curve was 0.903,the sensitivity was 82.8%,the specificity was 92.2%,the accuracy was 90.1%,and the Kappa value was 0.724.5 At the validation stage of clinical application value of simple assessment scale for progressive ischemic stroke,the sensitivity was 72.3%,the specificity was 89.6%,the accuracy was 84.1%,and the Kappa value was 0.628.Conclusions Simple Assessment Scale score was related to progressive ischemic stroke.The Simple Assessment Scale has a good effect on the assessment of progressive ischemic stroke,which is simple and convenient to use,and has a good clinical application value.Figure 2;Table 13;Reference 103...
