Objectives To verify the predictive value of simple assessment scale for ischemic stroke recurrence,so that it is convenient to be used in clinic. Methods Consecutively selecting 1288 patients with acute cerebral infarction admitted within 72 hours after onset during August,2015 to August,2017 in the nerval medical department ward of the Affiliated Hospital of North China University of Technology. In this study,a nested case-control study was designed.Patients were followed up for 6 months after discharge.During the follow-up period,Recurrence of ischemic stroke is the recurrence group(285 cases),or non-recurrent stroke group(1003 cases).General data of patients at admission were collected, and NIHSS score,MRS score and simple assessment scale were also scored.T test and variance analysis were used to Compare the measured data. The count data were analyzed by?~2-test.The changes of the scores of the two groups were compared, the correlation between the scores of the simple scale and the recurrence of ischemic stroke was analyzed. The predictive value of the simple scale for recurrence of ischemic stroke was further evaluated by drawing the working characteristic curve(ROC)of the subjects scored by the simple scale, and by paired chi-square test and consistency test(Kappa value). In addition,Consecutively collecting 366 patients with acute cerebral infarction admitted within 72 hours after onset during November,2017 to June,2018.They were admitted to nerval medical department ward. The validation and analysis of the predictive value of the Simple Assessment Scale for the recurrence of ischemic stroke were carried out. Finally, the predictive and discriminatory effect of the Simple Assessment Scale for the recurrence of anterior and posterior circulation ischemic stroke was discussed. Results 1 There was significant difference in the scores of simple assessment scale between relapse group and non-relapse group(P<0.05).2 When the score of the Simple Assessment Scale was divided into high group and low group with the mean value as the boundary point, the two groups were tested by chi-square test. The results showed that the composition of the score of the Simple Assessment Scale in the recurrence group was significantly different from that in the non-recurrence group(P<0.05).3 The results of multiple factor logistic regression analysis showed that there was a positive correlation between the score of the Simple Assessment Scale and the recurrence of ischemic stroke.4At assessment Stage of Predictive Value of Simple Scoring Scale for Ischemic Stroke Recurrence, the area under ROC curve was 0.918.the result shows as follows:sensitivity(89.8%),specificity(79.4%),accuracy(0.817),Kappa value(0.565).5 At the validation stage of the scale for predicting ischemic stroke recurrence, the result shows as follows:sensitivity was 0.986,specificity was 0.796,accuracy was 0.831,and kappa value was 0.580.6 The area under ROC curve was 0.941 for predicting the recurrence of anterior circulation ischemic stroke. The area under ROC curve was 0.902 for predicting the recurrence of posterior circulation ischemic stroke. The area was compared by Z test,Z value was 1.336(P>0.05),and there was no significant difference. Conclusions 1 Simple Assessment Scale score was associated with recurrence of ischemic stroke.2 The Simple Assessment Scale has certain value in predicting the recurrence of ischemic stroke. When the threshold value is 5,it has high sensitivity and specificity.3Simple Assessment Scale has no difference in predicting recurrence of anterior and posterior circulation ischemic stroke.Figure3;Table20;Reference 94...
