Objectives To observe the effect of simple tai chi training on the improvement of cardiopulmonary function and anxiety and depression in patients with stable chronic heart failure. To explore the safety and effectiveness of tai chi rehabilitation program,to provide more rehabilitation methods and approaches for patients with chronic heart failure in stable period, and to provide a new method for clinical cardiac rehabilitation nursing. Methods Using stratified sampling method, one service center(Shijiazhuang Dongjiao Subdistrict Office Community Health Service Center)was selected from 10 community health service centers contracted with family doctors in Hebei Provincial People's Hospital. From November 2018 to July 2019,80 patients with stable chronic heart failure with anxiety and depression were recruited in 6 communities under the jurisdiction of the community health service center. In order to avoid the contamination of the study subjects, the residential grouping method was used.Zhonghua street was used as the boundary, the study subjects to the west of zhonghua street were used as the control group, and those to the east of zhonghua street were used as the intervention group. The control group received regular community health education on chronic diseases,in addition to no exercise instruction. The intervention group was given simple tai chi training on the basis of health education in the control group,30?60 minutes each time,3?4 times a week, for a total of12 weeks.Tai chi chooses Yang's 24 style simple Taijiquan, the concrete step is the first step to warm up the exercise, the second step to carry on the simple tai chi training, the third step carries on the relaxation exercise.the cardiopulmonary function status of patients was measured before,4 weeks after and 12 weeks after intervention, including a sixminute walking test distance(6MWT),metabolic equivalent(METs),left ventricular ejection fraction(LVEF),forced expiratory volume(FEV1)and forced vital capacity(FVC)per second.the generalized anxiety scale(GAD-7)was used to measure the degree of anxiety of patients;the depression scale(PHQ-9)of patient health questionnaire was used to measure the degree of depression of patients.input all data into the EXCEL database and use SPSS17.0 software for statistical analysis of the data. The measurement data were described using mean ± standard deviation,t test, rank sum test and ANOVA;counting data were described by frequency(%),chi-square test;test level?=0.05.Difference with P<0.05 was deemed as statistically significant. Results This study completed data collection of 76 subjects, including 39 in the control group and 37 in the intervention group. The results are as follows: 1.Effects of simple tai chi training on cardiopulmonary function in patients with chronic heart failure at a stable stage: 1)there was no statistically significant difference in the indicators of 6MWT,METs and LVEF between the two groups before the intervention, which was comparable(P>0.05).After 4 weeks of intervention and 12 weeks of intervention, the indexes of6 MWT,METs and LVEF in the intervention group were significantly higher than those in the control group, with statistically significant differences(P<0.05).After 4 weeks of intervention, the difference of indicators of 6MWT,METs and LVEF between the two groups was statistically significant(P<0.05).After 12 weeks of intervention, the difference of indicators of 6MWT,METs and LVEF between the two groups was statistically significant(P<0.05).Repeated measurements showed that the time master effect, intervention master effect and time*intervention interaction effect in the indicators of6 MWT,METs and LVEF in the two groups were statistically significant(P<0.05).2)therewas no statistically significant difference in the FEV1 and FVC indexes between the two groups before intervention, which was comparable(P>0.05).There was no statistically significant difference in FEV1 and FVC indexes between the intervention group and the control group after 4 weeks and 12 weeks of intervention(P>0.05).After 4 weeks of intervention, there was no statistically significant difference in FEV1 and FVC between the two groups(P>0.05).After 12 weeks of intervention, there was no statistically significant difference between the two groups in FEV1 index difference(P>0.05).In terms of FVC index difference, the intervention group was significantly higher than the control group, with statistically significant difference(P<0.05).Repeated measurements showed that in terms of the indicators of FEV1 and FVC,the difference between the two groups was statistically significant(P<0.05)in terms of the time main effect, while the remaining effects were not statistically significant(P>0.05).2.Effects of simple tai chi training on anxiety and depression in patients with chronic heart failure in stable period: There was no statistically significant difference in the scores of GAD-7 and PHQ-9 between the two groups before intervention, which was comparable(P>0.05).The scores of GAD-7 and PHQ-9 in the intervention group after 4 weeks and 12 weeks were significantly lower than those in the control group, and the difference was statistically significant(P<0.05).After 4weeks of intervention, the difference of GAD-7 and PHQ-9 scores between the two groups was statistically significant(P<0.05).After 12 weeks of intervention, the scores of GAD-7and PHQ-9 in the two groups decreased significantly better than that in the control group(P<0.05).Repeated measurements showed that the two groups had statistical significance in the scores of GAD-7 and PHQ-9,and in the main time effect, the main effect of intervention, and the interaction effect of time*intervention(P<0.05).Conclusions Simple tai chi training can significantly improve the cardiopulmonary function, anxiety and depression of patients with chronic heart failure in stable period, and the longer the intervention time, the better the improvement effect.Figure 7;Table 23;Reference 147...
