Study On The Indexes Of Rational Use Of Glucocorticoidin Asthma Patients

Glucocorticoid is widely used in medical field,especially in clinical departments of hospital,which can give full play to the pharmacological effect of glucocorticoid and reduce adverse drug reactions is the focus of rational use of glucocorticoid drugs.At present,most hospital units in China investigate,evaluate or intervene on the clinical application of glucocorticoid drugs according to the guiding principles of glucocorticoids,the relevant guidelines or the detailed rules for the implementation of glucocorticoid drugs formulated by provinces and cities.However,there is no effective index system for evaluating the rational use of glucocorticoid drugs.In order to explore a set of evaluation system which includes the key links in the rational use of glucocorticoids in asthma patients,this study was carried out in order to comprehensively evaluate the rationality of glucocorticoid use in asthma patients.To improve the clinical rational use of glucocorticoid drugs,reduce the incidence of adverse drug reactions,improve the effect of drug therapy.Objective: to establish a set of evaluation indexes for the rational use of glucocorticoid drugs,so as to provide reference for the rational use of glucocorticoid drugs in patients with asthma in hospitals,and to promote the standardization and rationalization of the use of glucocorticoids in hospitals.Methods: in this study,according to the process of glucocorticoid use,the index system was divided into four categories: doctor,nurse,pharmacist and patient,and the index system for evaluating the rationality of glucocorticoid drug use in asthma patients was established.It includes4 first-level indexes: doctor index,pharmacist index,nurse index and patient index,7 secondary index and 14 third-level index.A total of 268 cases were selected by hospital electronic medical record system for real-time verification.84 patients were followed up and followed up on the whole process of glucocorticoid use.In addition,WeChat education and specialist education were adopted to comprehensively evaluate the patient indicators of the half-year intervention,and the ACT assessment,ICS inhalation technology mastery,patient compliance and patient satisfaction before and after the intervention were compared.Results: the three-level indexes were mainly verified,and the statistical results showed that the patient index data were significantly improved after the corresponding intervention measures were taken.The results of partial control statistics of ACT score in patients with mildasthma increased from 10.59% to 100.00%,and those in patients with severe asthma increased from 4.71% to 67.12%.The regular use of glucocorticoids in patients with mild and severe asthma increased from3.53% and 9.41 to 75.00% and 58.90%,respectively.The satisfaction of mild and severe asthma patients with glucocorticoid treatment increased from 7.06% and 57.65% to 75.00% and 89.04%.There was no significant difference in basic data between the conventional treatment group,WeChat treatment group and specialist treatment group(P>0.05).After the intervention,ICS inhalation technology mastery rate of the patients was compared,and the difference was statistically significant(P<0.05).Comparison of ICS treatment compliance between the three groups at 3and 6 months after intervention showed statistically significant differences(P<0.05).The difference of satisfaction before and after intervention was statistically significant(P<0.05).Conclusion: through the verification and evaluation of the preliminary index system,a set of key factors involved in the application process of glucocorticoid is established.A set of evaluation index system reflecting the rational use of glucocorticoid drugs in asthma patients was obtained.WeChat education and health education can improve the control level of asthma patients.Two intervention methods,WeChat education and health commissioner education,can enable asthma patients to fully master ICS inhalation technology and improve the ICS inhalation masteryrate and compliance.WeChat education and specialist education can improve patients' treatment satisfaction.
