With the development of medical technology and social economy,the proportion of preterm infants is increasing year by year.Premature infants have high mortality and sequela rates due to their immature organs,so how to improve the prognosis of preterm infants has become a growing concern in the medical community.Recent studies have shown that delayed cord clamping(DCC)can increase the newborn blood volume,increase the reserves of iron in the body,stable hemodynamics,improve neonatal immunity.At present,the research focuses more on term infants.With the increase of the proportion of preterm infants,the standardized operation of delayed cord clamping and the early impact on premature infants are becoming the focus of research.The purpose of this study is to provide a simple and effective method to improve the prognosis of preterm infants by observing the effect of delayed cord clamping on the early complications of preterm infants.Objectives:This study compared and analyzed the early complications of early cord clamping(ECC)and delayed cord clamping(DCC)of preterm infants,and explored the effect of delayed cord clamping on the early complications of preterm infants,so as to provide a simple and effective method to improve the prognosis of premature infants.Methods:A total of 101 preterm infants at 28-34 weeks who were born inevitably in our hospital from October 2017 to May 2019 and met the inclusion criteria were selected,and were divided into 2 groups according to the principle of randomized control: delayed cord clamping group(DCC group,53 cases)and early cord clamping group(ECC group,48 cases),and compare two groups of haemoglobin,haematocrit,neonatal hypoglycemia,neonatal jaundice,patent ductus arteriosus,periventricular-intraventricular hemorrhage,and collected relevant data and conducted statistical analysis with SPSS 21.0 statistical software.Results:The values of Hb and Hct in premature infants with delayed cord clamping group(DCC group)were higher than that of the early cord clamping group(ECC group)at 12 hours and 72 hours after birth(P<0.05);The incidence of hypoglycemia in preterm infants of DCC group was lower than that of ECC group within 72 hours after birth(P<0.05);Transcutaneous bilirubin(TCB)was significantly higher in DCC group than in ECC group on the first day after birth(P<0.05);T he TCB value of the DCC groups was higher than that of the ECC gro up from the second day to the seventh day after birth,but there was n o significant difference between the two groups(P>0.05),There was n o significant difference in the incidence of hyperbilirubinemia between t he two groups(P>0.05);The peak value of bilirubin in DCC group was higher than that in ECC group,There was no significant difference bet ween the two groups(P>0.05);The intervention rate of phototherapy inDCC group was lower than that in ECC group,but there was no signif icant difference between the two groups(P>0.05);There was no differe nce in the incidence of PDA and PIVH between the two groups(P>0.05).Conclusions:Delayed cord clamping can increase hemoglobin and hematocrit of preterm infants,so as to reduce the incidence of early anemia,reduce the incidence of hypoglycemia after birth,and not increase the incidence of jaundice,patent ductus arteriosus and periventricular-intraventricular hemorrhage.It is an economical and effective clinical treatment method.
