Objective: To assess the effects of delayed cord clamping in preterm infants.Methods:We searched through databases including Cochrane Library,Pubmed, Ovid, VIP citation databases,Wanfang database, CNKI,etc and retrieve randomized control trials(RCTs)on delayed cord clamping in preterm infants. The retrieved studies were screened by using inclusion and exclusion criteria. Methodological quality was evaluated by using Cochrane Handbook for systemnatic reviews and RevMan 5.1software. Meta-analysis was performed to extracted data by RevMan 5.1software.Result: seventeen randomized control trials were included.Meta-analysis showed the results followed:the blood pressure within 4hours after birth( WMD=2.49; 95%CI : 0.74~4.24),the hemoglobin concentration( WMD=15.92, 95%CI : 6.37~25.47) andthe hematocrit(WMD=4.84,95%CI:3.47~6.22) in 24 hours after birth in the delayed cord clamping group were higher than those in the early cord clamping group. The risk of anemia(RR=0.62,95%CI:0.47~0.81),IVH(RR=0.64,95%CI:0.45~0.91) and mortality(RR=0.42,95%CI:0.20~0.86)in the delayed cord clamping group were lower than those in the early cord clamping group. The peak of serum bilirubin, the risk of hyperbilirubinemia and polycythemia between the 2 groups did not showt significant difference.There were also no significant differernce in the need of blood transfusion and Phototherapy.Conclusion:Delayed cord clamping is safe to preterm infants as well as effective to improve the outcome of premature.
