| Objective To detect the distribution and expression of inducible costimulator(ICOS)and inducible costimulitor-ligand(ICOS-L)in different types of nasal polyps in order to study their roles in the pathogenesis of eosinophilic chronic rhinosinusitis with nasal polyps.Methods 29 patients diagnosed as chronic rhinosinusitis with nasal polys in the Affiliated Hospital of Qingdao University,who have received Functional Endoscopic Sinus Surgery(FESS),were selected as the nasal polys group,while another 8 cases of normal sinus mucosa including those receiving optic nerve decompression,extraction of intraorbital foreign body and repair of cerebrospinal fluid rhinorrhea were selected as the normal group.Lund-Mackay method was used to evaluate the illness severity of all patients.After HE staining of nasal polyp tissues,the nasal polys group was divided into the eosinophilic chronic rhinosinusitis with nasal polyps(ECRS)group and the non-eosinophilic chronic rhinosinusitis with nasal polyps(nonECRS)group according to the eosinophilic count.The distribution and expression of ICOS and ICOS-L in both groups were detected by immunohistochemical staining and Western-blot.RT-PCR was used for testing the m RNAs of ICOS and ICOS-L,then the relative quantitative analysis was conducted by △CT method.SPSS 24.0 software was used to analyze the data.Results 1.The average number of eosinophils per HPF in the ECRS group,non-ECRS group were: 59.93±23.79/10 HPF,3.93±2.6/10 HPF.2.Immunohistochemical staining showed that ICOS and ICOS-L were expressed in all three groups,presenting as brownish yellow,tan or light yellow particles on cell surface.Compared with the control group(ICOS score: 43.80±8.49,ICOS-L score: 63.78±26.01)and nonECRS group(ICOS score: 52.09±26.01,ICOS-L score: 73.77±26.82),the average score of ICOS and ICOS-L H-score was significantly higher(113.24±25.92 and 122.84±17.18,respectively,all P<0.01);while the expression of ICOS and ICOS-L between the ECRS group and the control group was not significantly different.3.Western Blot showed that the expression of ICOS in ECRS group is significantly higher than that in the non-ECRS group(1.48±0.22 vs 1.02±0.08),while there was no statistical difference between non-ECRS group and the Control group(1.02±0.08 vs 0.96±0.10).The gray scale ratio of ICOS protein in ECRS was 1.54±0.25,and that in non-ECRS was 1.19±0.23.The gray scale ratio of ICOS-L protein in the control group was 1.05±0.21.There was no statistically significant difference between the expression of ICOS and ICOS-L in non-ECRS and the control group(P > 0.05).4.RT-PCR used to detect the three groups.The △Ct values of ICOS m RNA in ECRS group,non-ECRS group and control group were 1.99±0.85、1.12±0.62、1±0.56 respectively.The △Ct values of ICOS-L m RNA in three groups were 2.25±1.11、1.09±0.58、1±0.51 respectively The expression of ICOS m RNA and ICOS-L m RNA in ECRS group was higher than that in non-ECRS group and control group.The expression of both were increased,and the difference between the non-ECRS group and the control group was on statistically significance(P<0.01)Conclusion The costimulation signals of ICOS and ICOS-L in ECRS were significantly higher than those in non-ECRS,indicating that ICOS and ICOS-L may play a potential role in the immune response of ECRS. |
