Objective:To investigate the predictive value of dynamic changes within 3 days after TEG after thromboelasticity(TEG)on the occurrence of thrombotic events in patients undergoing total knee arthroplasty. Methods :Retrospective analysis of 82 cases of total knee arthroplasty treated in our hospital from September 2018 to June 2019.The results of lower extremity venous ultrasound one day after operation revealed that 70 patients had no deep vein thrombosis of the lower extremity. There are 12 patients with deep vein thrombosis of the lower extremity.According to age, body mass index, and gender, matching was performed to obtain a 1: 1 thrombus group(N =10)and a non-thrombotic group(N =10).All patients were tested for thromboelastography, coagulation function, and other biochemical items before,on days 1 and 3 after surgery.Results:(1)On the first day after surgery, the α value of the thrombus group was significantly higher than that of the non-thrombosis group [69.75(68.08,72.43)VS.68.10(67.58,69.58)](P = 0.123),Ly30(P = 0.353),R value(P = 0.546),K value(P= 0.248)and MA value(P = 0.512)had no significant difference. There was no statistically significant difference in TEG values between the two groups before surgery and 3 days after surgery.(2)ROC curve analysis. The optimal critical value of α value at1 day after operation is 73.15 °,and the corresponding sensitivity, specificity, and area under the curve are 50%,100%,and 82.5%,respectively.Conclusion:In the short-term thrombus elasticity diagram after total knee arthroplasty, the α value of one day after surgery has a certain predictive effect on thrombosis events. When the patient’s α value is> 73.15 °on the first day after surgery,it must be paid attention by clinicians.Early clinical intervention can help the detection and timely diagnosis and treatment of venous thrombosis of the lower extremities.
