Abstract/Summary:
Objective 1.Correlation between the incidence rate of total DVT or symptomatic DVT and the risk factors of metabolic syndrome after total knee arthroplasty and total hip arthroplasty.2.Correlation of the incidence rate of total DVT or symptomatic DVT with the number of risk factors for metabolic syndrome after total knee arthroplasty and total hip arthroplasty.Methods A retrospective analysis was performed on 618 patients with primary unilateral TKA and 288 patients with primary THA in our department from January 2020 to December 2021.Met S was diagnosed according to the criteria of Diabetes Branch of Chinese Medical Association.At the same time,all patients were divided into 5 groups according to the number of Met S risk factors(0Met S factor group,1Met S factor group,2Met S factor group,3Met S factor group,4Met S factor group).All subjects were examined preoperatively by ultrasonography of the blood vessels of both lower extremities to exclude preoperative deep vein thrombosis,and also underwent ultrasonography of the blood vessels of both lower extremities after operation to screen for DVT.At the same time,clinical signs were examined and symptomatic DVT events after TJA were recorded.Univariate analysis and multivariate logistic regression analysis were used to clarify the correlation of Met S and its components with postoperative DVT and symptomatic DVT,and to explore the correlation between the number of Met S components and postoperative DVT and symptomatic DVT.Results A total of 618 patients who underwent TKA were included in this study,including 260 patients with postoperative DVT,accounting for 42.10%,and 49 patients with symptomatic DVT,accounting for 7.9%.In total knee arthroplasty,hyperlipidemia was associated with both total DVT[95%CI(1.492-3.310),P=0.000]and symptomatic DVT[95%CI(2.192-15.232),P=0.000].BMI≧25(kg/m~2)was associated with total DVT[95%CI(0.371-0.801),P=0.027],not with symptomatic DVT[95%CI(0.623-4.777),P=0.074],and Met S was associated with total DVT[95%CI(0.884-11.134),P=0.041]was not related to symptomatic DVT[95%CI(0.884-11.134),P=0.077].288 patients who underwent THA were included in this study,of whom 58 patients(20.10%)had postoperative DVT and 10 patients(3.50%)had symptomatic DVT.In total hip arthroplasty,hyperlipidemia during total hip arthroplasty was associated with both total DVT[95%CI(1.970-8.627),P=0.000]and symptomatic DVT[95%CI(1.529-30.232),P=0.020].BMI≥25(kg/m~2)was associated with total DVT[95%CI(0.165-0.783),P=0.010],but not with symptomatic DVT[95%CI(0.149-3.715),P=0.718].The prevalence of total DVT and the prevalence of symptomatic DVT increased with the number of Met S components in patients after THA and TKA,according to the chi-square test for trend.Conclusion 1.Met S patients are at increased risk of developing total DVT after total knee arthroplasty.Dyslipidemia in the Met S component is an independent risk factor for total DVT and symptomatic DVT after total hip and total knee arthroplasty;overweight(BMI≧25kg/m~2)is an independent risk factor for total DVT after total hip and total knee arthroplasty independent risk factor2.The incidence of postoperative total DVT and symptomatic DVT in patients undergoing total hip arthroplasty and total knee arthroplasty is related to the number of Met S components.The higher the number of Met S components,the higher the incidence of postoperative total DVT and symptomatic DVT.3.Analysis of clinical data found that age,dyslipidemia and metabolic syndrome were independent risk factors for the incidence of DVT after total knee arthroplasty,and dyslipidemia was an independent risk factor for the incidence of symptomatic DVT after total knee arthroplasty.Dyslipidemia and overweight are independent risk factors for the incidence of total DVT after total hip arthroplasty,and dyslipidemia is an independent risk factor for the incidence of symptomatic DVT after total hip arthroplasty.Incidence of total DVT and symptomatic DVT in patients with joint replacement surgery to maximize their prognosis.
