Background:Diabetic Kidney Disease(DKD)is one of the serious complications of type 2 diabetes mellitus(T2DM).With the rapid changes of modern social economy and lifestyle,the incidence of T2 DM in the world has increased year by year in recent years.DKD has become the main cause of end-stage kidney disease.Although the common clinical treatments methods for DKD can alleviate or even control the progress of DKD to a certain extent,the overall effect of the treatment is still not ideal.Sleeve gastroresection(SG)is an effective laparoscopic weight loss surgery with good weight loss effect,low incidence of complications,good food tolerance and short hospitalization time,so it has become the first choice for the clinical treatment of metabolic syndrome.Previous clinical studies have shown that SG operation can significantly improve the renal function of DKD patients,but so far the exact mechanism of SG operation to improve renal function has not been fully clarified.As one of the main pathogenesis,oxidative stress plays a vital role in the pathogenesis of DKD.Both acute and chronic kidney injury have elevated levels of oxidative stress during disease progression.So far,it has not been reported whether the improvement of renal function after SG involves oxidative stress mechanisms.The purpose of this study was to investigate the mechanism of renal function improvement after SG by observing the effects of SG on oxidative stress in the kidneys of type 2 diabetic rats,HE staining to observe renal pathological changes.Objective: To observe the changes of kidney structure and function of type 2 diabetic rats after sleeve gastrectomy and to investigate the role of oxidative stress mechanism.Methods:20 male SD rats were fed with a high-fat diet,and a method of intraperitoneally injecting the streptozoin in the abdominal cavity was used to induce the model of type 2 diabetes.17 successful induction models were divided into two groups: sham-operation(SO)group(n = 7)and sleeve gastrectomy(SG)group(n = 10),SO group and SG group were treated with sham-operation and SG operation,respectively.The body weight,fasting blood glucose(FBG),24-hour food intake and serum glucagon-like peptide-1(GLP-1)were measured at time points include before and 1,2,3 and 4 weeks after operation.The activities of malondialdehyde(MDA),catalase(CAT),glutathione peroxidase(GSH-Px)and superoxide dismutase(SOD)in renal tissue as well as serum creatinine(SCr),blood urea nitrogen(BUN)levels and 24-hour urinary microalbumin(UALB)were measured at the end of 4 weeks after operation,Compared with the SO group,the renal tissue structure of the SG group was significantly improved observed by HE staining..Results:The FBG in the SG group continued to decrease after the operation.The FBG in the SG group was lower than that in the SO group at 1 to 4 weeks after operation.(1 week after operation,P <0.05;2-4 weeks after operation,all P values were <0.001),GLP-1 levels in the SG group continued to increase after surgery,and the serum GLP-1 level in the SG group was higher than that in the SO group 1 to 4 weeks after surgery,and the differences were statistically significant(all P values were <0.001).At 4 weeks after operation,the serum BUN,SCr,and UALB in the SG group were lower than those in the SO group,and the differences were statistically significant(all P values <0.001).At 4 weeks after operation,the MDA level in kidney of SG group was lower than that of SO group,and SOD,CAT,and GSH-Px in kidney of SG group were higher than those of SO group.The differences were statistically significant(all P values were <0.001).Conclusion: The renal structure and function of type 2 diabetic rats after sleeve gastroresection are significantly improved.SG operation may restore the balance between ROS production and antioxidant capacity,reduce the level of oxidative stress in diabetic kidney,and thus play an important role in improving renal function.
