Objective:To evaluate the relationship between dust mite allergen sensitization and lung function and airway inflammation in patients,as well as the correlation between Fractional exhale nitric oxide(FeNO)level,pulmonary function index and severity in patients with atopic asthma and non-atopic asthma.Methods : A total of 104 asthmatic patients aged 18-65 years and 40 normal control patients were collected from the Peace Branch of General Hospital of Northern War Zone.According to the serum specific IgE concentration greater than or equal to level 2(table 1),there were two group:dust mite atopic asthma(AA)and non-atopic asthma(NAA).According to its allergy concentration level is divided into:dust mite level2,dust mite level 3,dust mite 4.FeNO and pulmonary function were detected.Results : 1.There was no significant difference in age,height,weight and gender distribution of subjects between the AA group,NAA group and the normal control group.Compared with NAA group and normal control group,AA group had higher FeNO level,total IgE level,blood eosinophils count and worse lung function test indicators(P<0.05),while there was no discrepancy between NAA group and normal group(P>0.05).2.In the AA group,there were striking differences in FeNO,blood eosinophils and total IgE between different concentrations and grades of dust mite allergens,and there were significant differences in level 2 and level 3,and level 3 and level 4 of dust mite allergens.Comparison of lung function indicators between groups showed no significant difference in FEV1/FVC?FEF25-75%pred level between dust mite level 2 patients and level 3 patients(P>0.05),however,there were marked distinction between dust mite level 3patients and level 4 pantients(P<0.01).FeNO was positively correlated with total IgE(r=0.444,P<0.01),blood eosinophils count(r=0.722,P<0.01),and negatively correlated with FEV1/FVC(r=-0.469,P<0.01)?FEF25-75%pred(r=-0.561,P<0.01).3.In the NAA group,there was no correlation between FeNO and total IgE,blood eosinophils count?FEV1/FVC?FEF25-75%pred(P>0.05).Conclusion:1.In asthma patients,dust mite allergen-sensitized patients showed higher levels of FeNO,total IgE,and blood eosinophil counts.FeNO is related to atopic status and has no direct correlation with changes in lung function indicators.In the absence ofevaluation of lung function indicators,it may lead to over-diagnosis of asthma.2.Allergens can increase airway inflammation and worsen lung function,and dust mite specific IgE has important clinical significance as a marker to assess the severity of asthma.3.Avoiding allergens and other environmental exposures factors can delay the progression of asthma symptoms.
