Objective: To study the clinical and imaging features of dementia patients with language barrier.Mothods: Twenty-five patients,aged 50 ~ 75 years,diagnosed as Primary progressive aphasia in the Department of Neurology,the first affiliated Hospital of China Medical University,were selected.all the subjects in the study group needed CT/MR and other auxiliary examinations.Under the guidance of neurologists,the MMSE and MOCA was completed,with an interval of at least 5 minutes between each scale.The MMSE and MOCA subitems of the included subjects were compared among different subtypes to evaluate the advantages and disadvantages of the two scales in the diagnosis and recognition of primary progressive aphasia.The results of CT/MR and SPECT were compared to analyze the sensitivity and specificity of CT/MR and SPECT in the diagnosis and recognition of primary progressive aphasia.Results: By comparing and analyzing the different imaging results of the same subjects,the CT/MRI of the four subjects showed only atrophy in hippocampus,parietal lobe and so on.SPECT could reflect the decrease of blood perfusion in left frontal lobe,left temporal lobe and parietal lobe angular gyrus,left thalamus and so on.By comparing the results of MMSE with the results of simple language ability Assessment scale,MMSE showed that the four subjects had defects in reading,writing,retelling and listening instructions.The simple language ability Assessment scale reflects the damage of word retrieval in sentence repetition and naming,but there is no obvious abnormality in single word understanding,sentence understanding,language fluency,sports language and grammar.Conlusion: MMMSE has no significant difference in total score and sub-score of logopenic type and semantic type,MOCA has significant difference in naming score of logopenic type and semantic type,and no significant difference in total score of other items.SPECT revealed more sensitivity and specificity in the diagnosis of PPA than CT/MR revealed anatomic abnormalities.
