Objective:To explore the incidence,serological characteristics and related risk factors of Occult hepatitis B virus Infection(OBI)in HIV patients with AIDS.Methods:We retrospectively collected hospitalized HIV patients at AIDS stage,from January 1,2015 to December 31,2018,in the infectious department of the first hospital affiliated to China medical university,who were no prior ART,with negative HBsAg,no past history of chronic hepatitis B and HBV medication.According to the results of HBV DNA detection by high sensitivity Polymerase Chain Reaction(PCR)confirmation,patients were divided into OBI group and non-OBI group.We got the prevalence of OBI.We also compared the differences between two groups including age,gender,clinical index and laboratory data.Factors with P<0.10 and important demographic characteristics were put into the multivariate regression model to evaluate the risk factors associated with OBI.Results:A total of 145 patients were enrolled including 18 OBI patients with the prevalence of 12.4%.There were 127 cases in the non-OBI group accounting for 87.6%.The prothrombin time in non-OBI group was longer than it in OBI group(P=0.01).The incidence of HBeAb+,HBcAb+,and HBsAb+/HBcAb+ in OBI group were higher than those in non-OBI group(44.4% VS 17.3%,P=0.013;72.2% VS 38.6%,P=0.010;55.6%VS 26.8%,P=0.017).The prevalence of HBcAb-/ HBsAb-in OBI group was lower than it in non-OBI group(5.5% VS 29.9%,P=0.043).Logistic regression analysis showed that only HBeAb+ was a risk factor associated with OBI(OR=4.127,95%CI:1.355-12.570,P=0.013).The rate of HBe Ab+ in OBI was 44.4%(8/18),while 17.3%(22/127)in non-OBI.Conculsions: The incidence of OBI in HIV patients with AIDS was 12.4%.HBV DNA levels are usually low,and HBeAb is a risk factor associated with the occurrence of OBI.
