Objective(1)Examine the differences of serum autophagy related factor Beclin-1level among different heart function classes of chronic heart failure(HF)patients,analyze the relationship between serum Beclin-1 level and heart funcion.(2)Discover the correlation between serum Beclin-1 level and left ventricular remodeling parameters LVEDd and LVMI,provide new ideas for clinical prevention and treatment of chronic heart failure.MethodSelect patients who were hospitalized in the department of Cardiology at Hunan provincial people’s hospital from January 1st 2019 to September 1st2019.Patients who had diagnosed heart failure by the 2018 China Heart Failure management guidelines criteria longer than 6 months and had classified as heart function class II-IV by the NYHA classification were the HF group.Patients without any history,symptom or laboratory evidence of heart failure and did not met the exclusive criteria were the control group.3 ml blood of all patients were drawn in the next morning after admission,use ELISA test to detect the serum Beclin-1 level of each patient.Basic clinical data,laboratory results,echocardiogram parameters and serum Beclin-1 level of each patient were collected during hospitalization.Difference between two groups was analyzed by single factor T test,Mann-Whitney-U test and chi-square test.For multi-group,one-way ANOVA and Kruskal-Wallis H test was used for intragroup comparisons.Spearman rank correlation analysis and linear correlation analysis was used for analysis the correlation between Beclin-1 and other parameters.Result1.There were total 105 hospitalized patients who were both satisfied the inclusive criteria and did not met the exclusive criteria enrolled in this trial,there were 74 patients enrolled in HF group,including 39 males and 35 females,with a mean age of 62.35±11.35 years,and 31 patients were enrolled in control group,including 17 males and 14 females,with a mean age of64.91±11.09 years.By the NYHA classification standard,HF group further divided into three groups(NYHA Class II group,NYHA Class III group and NYHA Class IV group).There were 24 patients in NYHA Class II group,21 patients in NYHA Class III group and 29 patients with NYHA Class IV group.2.The levels of serum Beclin-1 and NT-proBNP in HF group were higher than those in control group,LVEDd and LVMI in HF group were larger than control group and LVEF in HF group was lower than control group.The differences between them was statistically significant(P<0.05).3.The levels of serum Beclin-1 in NYHA Class IV patients were higher than those in NYHA Class II or NYHA Class III group patients,the difference between groups were both statistically significant(P<0.005).The levels of serum Beclin-1 in NYHA Class III patients were higher than those in NYHA Class II patients,difference between the two groups was statistically significant(P<0.005).4.The values of LVEDd in NYHA Class IV patients were higher than those in NYHA Class II or NYHA Class III patients,the difference between the two groups was statistically significant(P<0.005).The values of LVEDd in NYHA Class III patients were higher than those in NYHA Class II patients,difference between the two groups was statistically significant(P<0.005).The values of LVMI in NYHA Class IV patients were higher than those in NYHA Class II or NYHA Class III patients,the difference between the two groups was statistically significant(P<0.05,P<0.005).The values of LVMI in NYHA Class III patients were higher than those in NYHA Class II patients,difference between the two groups was statistically significant(P<0.005).5.Spearman rank correlation analysis shows the levels of serum Beclin-1in chronic heart failure patients have a significant correlation with their NYHAheart function class(r=0.535,P<0.001),and among other factors,LVEDd and LVMI have the strongest relationship with serum Beclin-1 level,the correlation among them was statistically significant(r=0.747,P<0.001,r=0.741,P<0.001).LVEF and age are also have a weak but significant correlation with Beclin-1(r=-0.490,P<0.001,r=0.269,P<0.001).6.One-dimensional linear regression shows the levels of serum Beclin-1were positively correlated with LVEDd and LVMI,the correlation was statistically significant(r=0.675,P<0.05;r=0.715,P<0.05).Conclusion1.There is a difference in serum Beclin-1 level,LVMI and LVEDd among chronic heart failure patients with different heart function classes,the correlation between serum Beclin-1 and heart function was significant in patients with chronic heart failure.2.Serum Beclin-1 level have a positive correlation with the severity of left ventricle structural reconstruction.3.The elevation of serum Beclin-1 level may have played a role as a contributing factor in chronic HF patient’s heart function degradation by mediating left ventricle structural reconstruction.
