With the acceleration of industrialization,urbanization,and aging,health risks of residents have been increased,and the number of chronic diseases has increased rapidly in China.Population data was released by the National Bureau of Statistics in early 2019 showed that by the end of 2018,the population over 60 years of age had reached 250 million,accounting for 17.9% of the total population,of which nearly180 million were chronic disease patients.In order to improve the quality and efficiency of medical and health services for various diseases such as chronic diseases,the World Health Organization proposed in 2007 to use integrated methods to eliminate the problem of broken and fragmented health services.The report of the Nineteenth National Congress of the CPC and the overall development outline of the health system both proposed the development of an integrated health system.In order to achieve the ultimate goal of building an integrated medical and health service system,we have explored an integrated medical and health service path that is in line with China's national conditions by developing a hierarchical diagnosis and treatment system and advancing the construction of medical associations.However,the overall development is slow,and the exploration is still in its preliminary stage.There is still a long way to go to provide people with health-oriented,comprehensive,and full-cycle services.The key nodes,important links and main problems in the process of integration are not clear.Therefore,this article takes the hypertension of chronic disease in district A of Shanghai as an example to collect and analyze the health data related to the disease.On this basis,the social network analysis method is mainly used to explore the breakpoints of the integrated medical and health service network,and to analyze the key issues that should be paid attention to,so as to solve the problem of the integration of medical and health services for chronic diseases such as hypertension in China,and it will provide the direction for health policy.First,the literature method was used to comprehensively sort out the research results and deficiencies of integrated medical services at home and abroad.Manyforeign countries have begun to use the method of social network analysis to study the integrated medical service of complex diseases,and many have achieved practical results.However,most of the domestic qualitative and qualitative studies have been conducted,and few researchers have explored with chronic diseases or complex diseases based on this method.This provides research space for the analysis of integration about services of hypertension and other complex disease between medical institutions by using the method of social network analysis in this article.Secondly,the relevant data of hypertension integrated services were collected and sorted,and a number of network research indicators were selected according to the research purpose,and a research plan was explored to explore the status of cooperation between medical institutions in district A of Shanghai.Transform the visits and referrals of patients with hypertension in the health information system into the admissions and referrals of medical institutions.The social network analysis method is used to carry out a comprehensive study of integrated medicine health service on the health data of medical institutions at all levels in district A.Thirdly,an empirical analysis of the integration of hypertension medical services was carried out.The social network analysis method is mainly used to carry out an in-depth and systematic analysis of degree centrality,feature vector centrality,intermediary centrality,cohesion and compactness.Indicator value calculations and visualizations are used to study the density of the overall medical service provider network and the closeness of medical service organizations at all levels.The qualitative,quantitative,and network diagrams were visually presented,the contribution and importance of each medical institution were clarified,and the key points to improve network tightness were identified.Finally,discussions were made based on the analysis results,and related policy recommendations were put forward.The results of the study show that: Area A shows a close connection as a whole,and there are also community health service centers that are obviously on the edge.The distribution of patients across medical institutions is uneven.Higher-level medical institutions bear many tasks in the provision of medical services,and their medical services are heavily burdened.The capabilities of community health service centers vary widely.According to the research results,improving the health information network and improving the basic medical professional service capabilities are the foundation,which is conducive to enhancing the interconnection and interconnection between medical institutions,breaking theburden of high-level medical institutions and the uneven distribution of patients.While establishing a health information system that covers the entire population,the idea of sharing information only among service providers should be updated,and more attention should be paid to the realization of information sharing between the demand side and the supply side.In the integration work,we must attach great importance to the idea of balancing the interests of all parties throughout the entire process of medical and integrated health service,and use incentives to make the integration of medical and health services work.
