ObjectivesThis study takes the construction of the Women and Children Specialist Association in Hechuan District and the construction of the Maternal and Child Health Consortium in Yongchuan District in Chongqing as a typical case.Based on the rainbow model and social network analysis,this paper analyzes the integration measures and inter-institutional collaboration of the maternal health service system in the two districts,discusses the highlights and problems in the process of integration of the two districts and provides policy suggestions for improving the integration of maternal and maternal health service system in the two districts.ContentsSummarizing integration measures of integrated health Service system for pregnancy and maternity in Chongqing Hechuan District and Yongchuan District,analyze the collaborative relationship between different agencies in the integrated maternal health service system of the two districts,discusses the highlights and problems in the process of cooperation.Information on the situation and the participants' attitudes towards the integrated maternal health service system.Relevant institutions will obtain relevant policy documents and related implementation plans,management systems,performance appraisal systems and other documents related to the construction of maternal health service system in the region.Based on the rainbow model,we will integrate the integration measures of system integration,organization integration,professional integration,service integration,functional integration and standardization integration in the integrated maternal health service system in the two regions;use social network analysis methods to analyze the integration of the two regions.Collaboration between agencies within the maternity health service system.Findings1.In terms of integration measures,Hechuan District and Yongchuan District will integrate the maternal and child health service system in the form of specialist alliances and maternal and child health unions,respectively,in system integration,organizational integration,professional integration,service integration,and functional integration.Corresponding measures have been taken in all aspects of standard integration.The two districts have certain commonalities in the integration measures.For example,at the level of system integration,the two regions have established corresponding work specifications,service processes and assessment systems;the service integration level,Hechuan District and Yongchuan District have been carried out to varying degrees.The obstetric grading referral system;at the level of functional integration,both regions have adopted integrated measures such as strengthening information construction,increasing financial input,and increasing assessment mechanisms for integration.However,there are still some differences in the specific integration measures between the two districts.For example,in terms of system integration,Hechuan District and Yongchuan District are led by government and district-level maternal and child health care institutions,and organizational integration is implemented.Hechuan District cancels primary obstetrics and establishes obstetric areas.Center,function integration,Yongchuan District has established a platform for women and children through the region.2.In terms of institutional collaboration,Hechuan District and Yongchuan District have certain similarities in referral networks,medical resource sharing,medical exchanges,mutual recognition of medical examination results,and health information sharing networks,all of which are presented at district-level maternal and child health centers.As the core,the network of maternal health service systems in the Unicom region,and the mode of connection between the district maternal and child health hospitals and some district-level secondary hospitals to municipal health institutions.On the referral network,Yongchuan District is more stable than Hechuan District;on the medical resource sharing network,the organizations in Hechuan District are more closely related,but the network rights and control structure of Yongchuan District are better;in medical communication,institutions at all levels in Yongchuan District The relationship is closer and more efficient,but Hechuan District People's Hospital is more involved in medical communication than Yongchuan;the mutual recognition of medical examination results,Hechuan and municipal medical institutions have more mutual recognition than Yongchuan,but they can achieve mutual recognition.The network is more closely connected;in the sharing of health information,Hechuan and the municipal medical institutions share less than Yongchuan,and the district people's hospitals participate less.The power and control structure of the health information sharing network in Yongchuan District is more obvious than Hechuan.DiscussionThe integration measures in Hechuan District and Yongchuan District have common features and their own characteristics,and there are certain similarities in the structure of referral network,medical resource sharing,medical communication,mutual recognition of medical examination results and health information sharing network..However,due to its integration measures and different priorities,Hechuan has its own strengths in different aspects.Hechuan is superior to Yongchuan in sharing medical resources and mutual recognition of inspection results.Yongchuan is better than Hechuan in referral,medical communication and information sharing.Although the two districts have optimized the maternal health service system to a certain extent,there are still certain problems.For example,the maternal grading guidance is not in place,and the related supporting mechanisms still need further improvement.SuggestionsIn order to further promote the construction of integrated maternal health service system and improve the quality of maternal health service,the following policy recommendations are put forward:first,to strengthen the guidance of graded diagnosis and treatment and two-way referral by using the role of medical insurance and price leverage;second,to build a human resources management system to strengthen the coordinated development of the system;third,to improve the distribution mechanism of relevant interests and enhance the system integration.Sustainability;Fourth,actively seek the balance of interests within the system,mobilize the integration enthusiasm of institutions at all levels.
