Objective This study builds an exercise program for elderly patients with type 2 diabetes with walking function based on evidence.By analyzing the current status of elderly patients with type 2 diabetes with walking function and systematic evaluation / meta analysis of exercise intervention for elderly patients with walking function,the evidence is summarized and evidenced.Hierarchy.Finally,through two rounds of expert consultation,the plan was revised and adjusted to form an evidence-based walking function exercise guidance program for elderly patients with type 2 diabetes.Methods The meta-analysis and systematic review and re-evaluation were used to integrate the best evidence to construct the first draft of the walking function instruction program for elderly patients with type 2 diabetes.The expert draft method was used to revise and adjust the first draft of the program to form the elderly type 2 patient.A walking function exercise program for diabetic patients.Results 1.Through a comprehensive and systematic search of Chinese and English databases,8 randomized controlled trials were finally included,and the quality of the literature was average or above.Classification and meta-analysis of the included literature show that exercise can improve the walking function of elderly diabetic patients,but the effect is not different when the exercise time is less than 5 months.Compared with other forms of exercise,aerobic exercise,It is more superior in improving the walking function of the elderly.Exercise can improve stride speed,endurance,lower limb strength,and lower limb mobility in elderly diabetic patients,but the effect on falls is unclear.2.Through a comprehensive and systematic search of Chinese and English databases,23 systematic reviews / meta analyses were finally included,and the quality of the literature was moderately high.The results showed that the effect of exercise intervention on the elderly’s walking function was controversial.Fourteen studies included in 23 literatures showed that exercise intervention could significantly improve the elderly’s walking function,and the other nine results showed no significant difference.According to the evidence,it is recommended that:(1)exercise should be performed under the guidance of a professional medical staff or physical therapist during exercise;(2)Tai Chi,Yoga,and Pilates are conducive to improving the balance and flexibility of the elderly.It has obvious advantages in posture;(3)It is recommended to include two or more kinds of exercise content,such as resistance,balance,flexibility and endurance exercises;(4)there is no evidence that pure resistance training can improve walking;(5))Exercise under the supervision,group or home exercise is more effective;(6)listening to music or rhythmic training is more effective to improve step;(7)at least 150 min per week(such as 5 d per week,30 each time Min)aerobic exercise of moderate intensity(80% of maximum heart rate,a bit hard during exercise,rapid heartbeat and breathing but not rapid).(Even a short-time physical exercise)aerobic exercise;(8)2 to 5 exercises per week,the interval between two exercises should not exceed two days.3.Adopt two rounds of expert correspondence.In the first round of expert correspondence,the average value of the importance index of 7.11 and 7.12 indicators is<3.5.According to expert recommendations,the 7.11 and 7.12 indicators are revised and adjusted.The remaining indicators are> 3.5.The second round The averages are all> 3.5;the coefficients of variation of 7.1,7.3,7.11,7.12 in the first round of expert consultation are> 0.25,and are modified and adjusted according to expert opinions and suggestions.The coefficients of variation of the remaining indicators are all <0.25,and the coefficient of variation in the second round Both are <0.25;the coordination coefficient W of the first round of expert consultation is 0.329,the P value is <0.001,the second round of W is 0.278,and the P value is <0.001,indicating that the expert opinions tend to be consistent and have good coordination.After two rounds of expert consultation,the sports plan was finally formed.Conclusion In this study,a comprehensive search of Chinese and English databases,integration of the best evidence,and expert consultation was used to construct a scientific and standardized walking function exercise guidance program for elderly 2 diabetic patients,including applicable subjects,exercise contraindications,instructors,and pre-exercise.Evaluation,warm-up before exercise,exercise form,exercise intensity,exercise frequency,etc.The program is based on evidence and can provide patients with scientific and standardized exercise guidance.
