| Objective:To know Hefei’s community maternal and child health(MCH)personnel’s knowledge,attitude and behavior status and training situation about early health management.We explore the main influencing factors of knowledge,believe and practice level of early life health management of community MCH personnel as well as the specific problems in training,specific training needs of community MCH personnel,aiming at providing a more targeted community maternal and child health personnel training and better promote community life early health management work provides the basis.Methods:In this study,Hefei city was selected as the research site,and the self-designed questionnaires about"knowledge,believe and practice"and training needs survey of community maternal health personnel and community child health personnel in early life health management were adopted.We went to the maternal health care department and child health care department of 52 community health service centers in Hefei city to investigate the actual on-the-job maternal health personnel and child health personnel in each center,including the basic personal information,knowledge,attitude,behavior,learning and training of health management in maternal/0-2-year-old children.Epi Data3.1 and SPSS 25.0 were used for the data.Using frequency,rate,composition ratio,mean,standard deviation,median and other indicators for general descriptive statistics,using chi-square test test method and multi factor Logistic regression method for influencing factors analysis.Results:(1)219 questionnaires were distributed in this survey,217 of which were valid,with an effective response rate of 99.1%.101(46.5%)maternal health personnel and 116(53.5%)child health personnel were investigated.In terms of gender distribution,there are 9 males(4.1%)and 208 females(95.9%);the age is between 21-64 years,the average age is(34.52±7.84)(median:34 points);the educational background of the respondents is mainly undergraduate(57.1%)and junior college(35.0%);the majors of the respondents are mainly clinical medicine(55.3%)and Nursing(30.0%);201 respondents(92.6%)In the survey,150 doctors(69.1%)and 67 nurses(30.9%)participated in the training of maternal health/0-2-year-old children’s health professional knowledge.Among them,doctors(65.3%)have bachelor’s degree,nurses(55.2%)have college degree,142(65.4%)had doctor’s qualification,65(30.0%)had nurse’s qualification and10(4.6%)had no qualification;In terms of the distribution of professional titles,primary and intermediate professional titles are the main ones(89.4%);187 full-time personnel(86.2%)and 30 part-time personnel(13.8%);86 formal personnel(39.6%)and 131informal personnel(60.4%);the monthly income of the respondents is mainly from 2000-4999 yuan,a total of 161(74.2%);the shortest period of working in MCH is insufficient1 year,the longest 38 years(median 4 years).(2)The results show that community maternal health personnel score 37-67(median 41)in maternal health management knowledge,with an excellent rate of 44.6%;the attitude score is 53-75(median 70),the excellent rate is 64.4%;the behavior score is 25-44(median 41),the excellent rate is66.3%;the total score is 143-183(median 170),the excellent rate is 59.4%.The total level of knowledge,believe and behavior of the full-time personnel are better than that of the part-time personnel(?~2=10.910,7.526,P<0.05),and the level of the doctors’health management behavior is better than that of the nurses(?~2=5.002,P<0.05).(3)The score of 0-2-year-old children’s health management knowledge of community child health care personnel is 29-64(median 53),the excellent rate is 25.0%;the score of attitude is 22-70(median 62),the excellent rate is 40.5%;the score of behavior is 63-112(median 100),the excellent rate is 46.6%,the total score of knowledge,attitude and behavior is 168-236(median 214.5),the excellent rate is 42.2%.The general level of knowledge,believe and behavior of children aged 0-2 years old of informal personnel are better than that of formal personnel(?~2=4.073,P<0.05).Multivariate analysis showed that the knowledge level of informal personnel is better than that of formal personnel(OR=4.296,95%CI:1.693-10.899,P<0.05).The attitude level of men is better than that of women(?~2=5.621,P<0.05).(4)The average annual training frequency of community MCH personnel is 0-2times(49.8%),the number of people expected to participate in 3-5 times(57.6%);the organization of training is the most district level health department(89.6%);the number of people participating in training is the most"centralized teaching"(95.0%),and the most expected form of training is"going to the superior hospital for exchange and further education"(74.2%).In terms of training content,the training content of the largest number of Community maternal health personnel is"knowledge of pregnancy health examination"and"knowledge of pregnancy disease screening and prevention"(90.6%),and the training of"knowledge of pregnancy disease screening and prevention"(84.2%)is expected to receive the largest number of people;the training content of the largest number of community child health personnel is"knowledge of pregnancy disease screening and prevention"(84.2%)The training content was"infant feeding knowledge"(92.4%),and the training content expected to receive the largest number of people was"infant disease prevention knowledge"(85.3%).In terms of problems in training,"training time is not free"(60.7%),"training form is too single"(53.7%)and"training frequency is insufficient"(38.3%)are the top three problems.In terms of training suggestions,"the form of training should be more diversified"(81.6%),"the content of training should be practical and timely updated"(69.1%)and"the schedule of training should be more flexible"(68.7%)put forward three suggestions with the highest frequency for the respondents.Conclusion:This study found that the age structure,educational background structure and the proportion of full-time and part-time jobs of respondents are generally good,but the professional title structure is not reasonable,high-level talents are insufficient,and the proportion of nursing personnel is insufficient.The overall knowledge level of early life health management of respondents is not good,the professional knowledge is not systematic and comprehensive enough.In terms of attitude,the health management attitude of the subjects in early life is good,but there is a certain degree of job burnout.In terms of behavior,the overall behavior level of the respondents is good,but some specific work still needs to be further improved.In terms of personnel training,there is a certain deviation between the content and form and the actual needs of personnel.The training time is not free,the training form is single,and the training content is outdated,which is the prominent problem in the training.In order to improve the knowledge and practice level of early life health management and the ability of early life health management and service,the government,society,community health service institutions hand community MCH personnel need to work together. |
