Objective: The purpose of this study was to analyze the related influencing factors of hyperuricemia(HUA)in patients with type 2 diabetes(T2DM)and to explore the relationship between triglyceride glucose index(TyG)and HUA.Method: From March 2018 to February 2019,298 T2 DM patients were diagnosed and treated in the Endocrinologl of Shaanxi Provincial People's Hospital.According to the presence or absence of HUA,it was divided into 89 cases of hyperuricemia group(HUA Group)and 209 cases of normal uric acid group(NUA Group).Collect the gender,age and course of all subjects;measurement of brachial artery diastolic pressure(DBP),systolic pressure(SBP),weight and height,and estimate the body mass index(BMI)=weight(kg)/ height(m2);use automatic Biochemical analysis to detect total cholesterol(TC),triglyceride(TG),Serum uric acid(SUA),Serum creatinine(SCr),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C);Use glucose oxidase method to detect fasting blood glucose(FPG);use high performance liquid chromatography to detect glycated hemoglobin(HbA1C),and estimate triglyceride-glucose index(TyG)TyG = ln [fasting TG(mmol / L)× FPG(mmol / L)/2].Using SPSS20.0 statistical software,and using t test,chi-square test,Pearson correlation analysis,Logistic multi-factor stepwise regression and other methods for data analysis,P<0.05 indicates that the difference is statistically significant.Result:1.Comparison of clinical data between two groups of patientsCompared with the NUA group,HUA group BMI,SBP,DBP,TG,SCr,LDL-C,TyG were significantly increased,the difference was statistically significant(P <0.05).Compared with the NUA group,the HUA group had lower age,HbA1 C,HDL-C,and the difference was statistically significant(P<0.05).Compared with the NUA group,the proportion of men in the HUA group is higher,and the difference is statistically significant(P<0.05);No obvious difference was found in the course of disease,TC and FPG between the two groups.2.Correlation analysis of SUA level and clinical indicatorsSUA was positively correlated with BMI,SBP,DBP,SCr,FPG,TG,LDL-C,TyG,and negatively correlated with age,HbA1 C,HDL-C,the difference was statistically significant(P<0.05),and UA and Significant association between disease course and TC.3.Comparison of incidence of T2 DM combined with HUA among groupsQ1,Q2,Q3,and Q4 subgroups were compared,and the difference was statistically significant(P<0.05).The incidence of T2 DM combined with HUA in Q4 subgroup was significantly higher than that in Q1,Q2,and Q3 subgroups(?2=12.867,11.201,8.886,P=0.0003,0.0008,0.0029),the difference was statistically significant(P<0.05).4.Multiple logistic regression analysis of the risk associated with TyG and T2 DM combined HUAAfter adjusting for gender,age,HbA1 c,SCr and other factors,the OR(95%CI)of TyG was 1.51(1.08,2.12)(P=0.017),the difference was statistically significant(P<0.05).Compared with the Q1-Q3 subgroup,the risk of HUA in T2 DM in the Q4 subgroup was significantly increased.After adjusting for multiple factors,the OR(95% CI)was 3.33(1.16,9.56)(P=0.025),the difference was statistically significant(P<0.05).Conclusion:1.In T2 DM patients,HUA is closely related to gender,age,obesity,dyslipidemia,hypertension,renal function,and TyG.2.T2 DM patients with HUA lipid metabolism disorder mainly manifested as increased TG and LDL-C,and decreased HDL-C.3.In T2 DM patients,TyG is positively correlated with SUA,and TyG is independently positively correlated with the risk of HUA,suggesting the importance of controlling blood sugar and blood lipids to reduce insulin resistance(IR)and prevent HUA.
