The continuous implementation of electronic medical records sharing and Internet plus medical care has made the progress of medical information and the risk of leakage and abuse of electronic medical records containing a large number of patients' privacy.The sharing and transmission format of(Electronic Medical Record,EMR)is different from the traditional structured data,in which the document segment of EMR has high reusability in the medical record and the content reflecting the patient's health privacy is described in the form of free text.This paper discusses the privacy protection under fine-grained access control for personal sensitive information in semi-structured electronic medical records.Through automatic identification and classification of sensitive words of medical text data,this paper proposes and implements a combination of attribute encryption and searchable encryption over EMR fine-grained privacy protection solution.The specific research work is as follows:(1)Aiming at the problem of automatic identification of personal health sensitive attributes in the sharing of electronic medical records,an algorithm is designed for the extraction,recognition and classification of patient health sensitive information in electronic medical records text,realizing the automatic accurate identification and classification technology of sensitive words of medical entities in electronic medical records,laying the foundation for the fine-grained privacy protection scheme of sensitive information of electronic medical records.(2)For the requirements of hierarchical classification and protection of privacy sensitive information in electronic medical records,based on the designed privacy recognition and classification algorithm,combined with attribute encryption andsearchable encryption algorithm,a selective encryption and confidential search scheme for electronic medical record sensitive information was designed and implemented ensuring the confidentiality of sensitive information,and achieving the availability of electronic medical record query,and basically solving the problem of access control granularity of electronic medical records.(3)Based on the aforementioned research,by the investigation of the shared security requirements of the medical records of the Sixth Hospital of Guiyang City,according to the software design and development process,an EMR privacy protection system was developed and implemented The system implements fine-grained access control of electronic medical records and confidential query of sensitive words.Running tests show that the system can meet the functional requirements and design goals,and has the value of further application and promotion.
