Purpose:By comparing the cerebral infarction recurrence rate and cerebral hemorrhage conversion rate of two groups of patients with arteriolar occlusive(SAO)cerebral infarction with or without cerebral microbleeds(CMBs),and comparing the influencing factors of cerebral infarction recurrence and cerebral hemorrhage conversion,the safety of antiplatelet aggregation therapy for patients with arteriolar occlusive(SAO)cerebral infarction was discussed.Methods:From July 2016 to May 2018,96 patients with acute arteriolar occlusive cerebral infarction who were treated in the First People's Hospital of Pingyuan County were selected for study.According to whether multi-modality magnetic resonance susceptibility weighted imaging(SWI)is combined with cerebral microbleeds(CMBs)at admission,the patients are divided into microbleeds group and non-microbleeds group,48 patients in each group,and patients combined with cerebral microbleeds(CMB)group are layered according to the amount of microbleeds,wherein 1-4 lesions are grade 1 and 5-9 lesions are grade 2.Ten or more patients were grade 3.All patients recorded baseline data,followed up for more than 6 months,and recorded the prognosis of patients,including cerebral infarction recurrence,cerebral hemorrhage transformation,etc.Results:6 patients were enrolled in the group,48 in the cerebral microbleeds group,including 20 in grade 1,15 in grade 2,13 in grade 3,and 48 in the non-cerebral microbleeds group.after a follow-up of more than 6 months,there were 15 cases of ischemic stroke recurrence,including 7 in the non-cerebral microbleeds group and 8 in the cerebral microbleeds group.comparative tests between the two groups showed that cerebral microbleeds(CMBs)had a correlation with ischemic stroke recurrence P<0.05,and there was no clear correlation between the two.There were 14 cases of cerebral hemorrhage transformation(including 0 cases of fatal cerebral hemorrhage and 14 cases of mild cerebral hemorrhage transformation),including 4 cases in the non-microbleed group and 10 cases in the microbleed group.the comparison between the two groups was P<0.05.the existence of microbleeds was clearly related to the transformation of cerebral hemorrhage.through multivariate analysis,correcting the grade of microbleeds(CMB)after other confounding factors was the independent risk factor affecting the transformation of cerebral hemorrhage(p<0.05).Conclusion:1.There is no obvious correlation between the presence of cerebral microbleeds and the recurrence of cerebral infarction in patients with arteriolar occlusive cerebral infarction.2.The existence of cerebral microbleeds is closely related to cerebral hemorrhage transformation of arteriolar occlusive cerebral infarction.The more cerebral microbleeds,the higher the incidence of cerebral hemorrhage transformation after infarction.
