The State Of Preoperative Anxiety And Its Relationship With Postoperative Recovery In Patients With Outpatient Hysteroscopy Surgery

Objective: This study aims to investigate the status and influential factors of the preoperative anxiety and to explore the relationship between preoperative anxiety and postoperative recovery in patients with outpatient hysteroscopic operation so as to provide theory basement and reference for nurses to design intervention of improving preoperative anxiety and promoting the quality of recovery.Method: This cross-sectional study selected 315 patients in outpatient hysteroscopy through convenient sampling.State Anxiety Inventory,Trait Anxiety Inventory,Medical Coping Modes Questionnaire and self-designed outpatient hysteroscopic surgery patients general information questionnaire were filled out by patients under being introduced.Patients' heart rate and blood pressure were measured before operation and the results were recorded in the patient's preoperative condition and postoperative recovery registration form.And then the patients' condition of preoperative anxiety was evaluated.The influencing factors of preoperative anxiety were explored through multiple linear regression analysis.At 15 min after surgery,patients' pain was evaluated by using numerical rating scale and patients' heart rate and blood pressure were measured.The condition of postoperative recovery was recorded including postoperative nausea,vomiting,dizziness,headache,postoperative recovery to preoperative comfort level,analgesic demand,and postoperative bed rest time and the type of diseases,the time of surgery,the type of operation were also recorded after surgery.And then all the postoperative results were recorded in the patient's preoperative condition and postoperative recovery registration form so as to evaluate the condition of postoperative recovery.Using correlation analysis and multiple linear regression analysis analyses the correlation between preoperative state anxiety and postoperative recovery as well as the impact level of preoperative anxiety on postoperative recovery.Results: 315 patients were enrolled and 314(99.68%)patients were analyzed.The average score of preoperative State Anxiety Inventory was 40.85±10.47,which was higher than the domestic norm(P<0.01),suggesting that patients with hysteroscopic surgery had anxiety before surgery.The influencing factors of preoperative anxiety included the degree of education,relationship with family members,history of hysteroscopic surgery,quality of sleep before surgery,yield coping style and trait anxiety through univariate analysis(P<0.05).Multiple linear regression analysis indicated that the main influencing factors of preoperative anxiety were the degree of education,quality of sleep before surgery,yield coping style.The score of preoperative State Anxiety Inventory was positively correlated with the score of yield coping style and was negatively correlated with the quality of sleep before surgery and the degree of education.The regression mode could explain 19.60% of the degree of variation in preoperative anxiety.The patients' main operation was hysteroscopic polypectomy(69.70%).15.00% of patients recovered to preoperative comfort level and 3.20% of patients had analgesic needs.The proportion of patients who developed nausea was 32.20% and the proportion of patients with postoperative vomiting was 9.20%.66.20% of patients had dizziness after operation and 1.30% of patients had postoperative headache.The median time of operation was 16.00 min.The mean heart rate was 73.50 beats/min 15 min after operation.The mean systolic blood pressure was 124.08 mmHg at 15 min after operation and the mean diastolic pressure was 72.38 mmHg at 15 min after operation.At 15 min after operation,the lowest pain score was 0.00 point and the highest was 9.30 points and the median pain score was 2.00 points.The median postoperative bed rest time was 23.00 minutes.The operative anxiety had an effect on the postoperative 15 min heart rate and the postoperative 15 min pain score in the postoperative recovery index(P<0.05)and had no effect on other postoperative recovery indexes(P>0.05).The preoperative anxiety was positively correlated with the heart rate at 15 min after operation and the correlation coefficient was 0.13(P<0.05).The preoperative anxiety was positively correlated with pain at 15 min after surgery and the correlation coefficient was 0.22(P<0.01).Multiple liner regression analysis found that the main influencing factors of pain at 15 min after operation were the systolic blood pressure at 15 min after surgery,the score of preoperative State Anxiety Inventory,the heart rate at 15 min after surgery and the time of operation.Preoperative anxiety was the main predictor of pain score at 15 min after surgery.Preoperative anxiety had a correlation with the postoperative recovery.Conclusion: Patients have an obvious anxiety status before outpatient hysteroscopic surgery.The preoperative anxiety is mainly influenced by trait anxiety,education level,sleep quality before surgery and yielding coping style.The higher score of the patient's preoperative State Anxiety Inventory is,the faster the heart rate is and the higher the pain score is.The higher the preoperative anxiety score is,the lower the quality of postoperative recovery is.The preoperative anxiety has a correlation with patients' postoperative recovery.
