OBJECTIVE:This study used a questionnaire survey to understand the status of self-efficacy,psychological resilience and postoperative rehabilitation in patients with lumbar disc herniation(LDH),analyze the influencing factors,and explore the correlation between self-efficacy,psychological resilience and postoperative rehabilitation in LDH patients,To provide a theoretical basis for clinical nursing staff to improve the patient's postoperative rehabilitation effectMETHODS:In this study,patients with LDH surgery in a Chinese medicine general hospital and a western medicine general hospital in Harbin were selected as the research subjects.The general data questionnaire,chronic pain self-efficacy scale(CPSS),and psychoelasticity scale(CD-RISC),Visual Analogue Scale(VAS)and Japanese Orthopaedic Association Low Back Pain Scale(JOA)were investigated at 1 day before operation,2 weeks after operation,and 3 months after operation to analyze the self-efficacy and psychology of patients Resilience,the status quo of postoperative rehabilitation and influencing factors,and explore the relationship between self-efficacy,psychological resilience and postoperative rehabilitation in LDH patients.RESULT:1.A total of 360 questionnaires were distributed in this survey,excluding 49 questionnaires that did not meet the conditions,and finally 311 questionnaires were valid,with a questionnaire validity rate of 86.4%2.LDH patients' self-efficacy and influencing factors The average score of LDH patients' self-efficacy was(65.32 ± 17.162),which was in the middle level.Among them,male,age<45 years old,education level is bachelor degree or above,residence is urban,per capita monthly income of family is?3000 yuan,pain duration is less than 3 years,mild pain,patients with other chronic diseases have similar self-efficacy levels Higher.The main influencing factors of LDH patients' self-efficacy are:pain time,preoperative pain level,whether they have other chronic diseases and education level.3.LDH patients' psychological resilience and influencing factorsThe average psychological resilience score of LDH patients is(59.59±14.331),which is lower than that of adults.Among them,patients with age<45 years old,education level of bachelor degree or above,city of residence,average monthly household income ?3000 yuan,medical insurance,pain duration<3 years,mild pain,no other patients with other chronic diseases Relatively highThe main influencing factors of psychological resilience in patients with LDH are:pain time,preoperative pain level and education level4.Postoperative rehabilitation effect and influencing factors of LDH patientsThe JOA scores of LDH patients 1 day before operation,2 weeks after operation,and 3 months after operation were(12.41±3.536)points,(18.8±2.420)points,and(23.69±1.721)points,showing a significant upward trend Wound infection occurred in 5 patients at 2 weeks postoperatively.Nine patients were found to have lumbar spine instability and symptoms such as lumbar pain and numbness of lower extremities after 3 months of postoperative reviewThe main influencing factors of the recovery effect of LDH patients after 2 weeks are:whether they have other chronic diseases,the time after getting out of bed for the first time,the time of pain,the degree of pain before surgery,the degree of pain at 2 weeks after surgery,psychological flexibility,and self-efficacy.The main factors influencing the recovery effect of LDH patients after 3 months are:whether they have other chronic diseases,pain time,pain level at 3 months after operation,psychological flexibility,and self-efficacy.5.The relationship between self-efficacy,psychological resilience and postoperative rehabilitation of LDH patientsThe self-efficacy of LDH patients was positively correlated with the recovery effect at 2 weeks and 3 months after operation(r=0.564,0.523),and the psychological elasticity was also positively correlated with the recovery effect at 2 weeks and 3 months after operation(r=0.531,0.463)The mediating effect of mental resilience between self-efficacy and the recovery effect at 2 weeks and 3 months after surgery accounted for 22.5%and 20.3%of the total effect,respectivelyCONCLUSION:1.The overall self-efficacy of LDH patients is at a medium level.It is recommended that clinical medical personnel attach importance to the cultivation of positive emotions of patients to further improve their self-efficacy level2.The psychological resilience of LDH patients is lower than that of adults.It is recommended that clinical medical personnel carry out some targeted psychological interventions on the patients.At the same time,the family and children of the patients must also accompany the patients frequently to improve the patients' psychological resilience3.The postoperative rehabilitation effect of LDH patients is ideal,mainly affected by the presence or absence of other chronic diseases,pain time,postoperative pain level,psychological resilience and self-efficacy4.The self-efficacy and psychological resilience of LDH patients are positively correlated with the postoperative rehabilitation effect.At the same time,the psychological resilience serves as a partial intermediary between the self-efficacy and the recovery effect of 2 weeks and 3 months after surgery,which can be used as postoperative rehabilitation for patients Protective factors.In order to optimize the postoperative rehabilitation effect of LDH patients,it is necessary to actively take scientific and effective intervention measures to improve the patients' self-efficacy level and enhance their psychological resilience.
