Objective To explore the correlation between high density lipoprotein cholesterol(HDL-C)and cystatin C and brachial-ankle pulse wave velocity(baPWV)in patients with essential hypertension,in order to provide an effective monitoring index for the auxiliary diagnosis and management of essential hypertension.Methods 1.A total of 131 adults inpatients and outpatients with essential hypertension diagnosed in the First Affiliated Hospital of Medical University Of Anhui from April2016 to August 2018 were randomly collected as the essential hypertension group.There were 70 male and 61 female with the average age of(53.69 ± 9.06)years.And according to the difference of the blood pressure level,the patients in the essential hypertension group were divided into essential hypertension group I,II and III.At the same time,110 healthy people [60 male and 50 female,and their average age was(50.67 ± 9.09)years old ] were collected as the control group in this study.Serum samples of all subjects were collected.2.The direct method-catalase removal method was used to detect the level of serum HDL-C.3.Latex turbidimetry was used to detect the level of serum cystatin C.4.Brachial-ankle pulse wave velocity was measured by the Omron VP-1000 Automatic Arteriosclerosis Measurement System.5.SPSS 18.0 software was used for statistical analysis.Measurement data wereexpressed as mean ± standard deviation.T-test or anova were used for inter-group comparison,count data were expressed as number(n)or rate(%),and the chi-square test was used for inter-group comparison.Pearson correlation analysis was used for assessing correlation between HDL-C,cystatin C and baPWV.Multiple stepwise regression analysis was used to analyze the related factors of hypertension.P<0.05 was considered statistically significant.Results 1.Baseline data study: compared with the control group,the patients of the essential hypertension group had higher values of age(P=0.011),smoking status(P=0.000),alcohol drinking status(P=0.000),heart rate(P=0.000),fasting blood glucose(P=0.000),total cholesterol(P=0.048),uric acid(P=0.019),creatinine(P=0.016),homocysteine(P=0.002),c-reactive protein(P=0.002).No significant differences of BMI,trigly-ceride and LDL-C existed between the two groups(all P>0.05).2.HDL-C in the essential hypertension group was significantly lower than that in the control group,and the difference was statistically significant(1.31 ± 0.32 mmol/L vs1.44 ± 0.35 mmol/L,P=0.004).Cystatin C and baPWV were significantly higher in the essential hypertension group than in the control group(0.81 ± 0.22 mg/L vs 0.71 ± 0.19mg/L;1724.72 ± 349.21 cm/s vs 1332.68 ± 212.74 cm/s).All the differences were statistically significant(all P<0.05).3.The levels of HDL-C in hypertension group 1,group 2 and group 3 were lower than that in the control group(all P<0.05).The levels of HDL-C in hypertension group 3 was lower than hypertension group 2 and group 1(all P<0.05).The levels of HDL-C in hypertension group 2 was lower than group 1(P<0.05).4.The levels of cystatin C and baPWV in hypertension group 1,group 2 and group 3were higher than that in the control group(all P<0.05).The levels of cystatin C and baPWV in hypertension group 3 were higher than hypertension group 2 and group 1(allP<0.05).The levels of cystatin C and baPWV in hypertension group 2 were higher than group 1(P<0.05).5.According to results of Pearson correlation analysis,HDL-C is negatively correlated with cystatin C(r=-0.167,P=0.009),and negatively correlated with baPWV(r=-0.190,P=0.003),and cystatin C is positively correlated with baPWV(r=0.359,P=0.000).6.Multiple stepwise regression analysis presented that hypertension independently related to baPWV,fasting blood glucose and heart rate(?=0.005,1.248,0.054;P=0.000,0.005,0.011).Conclusion Serum HDL-C in patients with essential hypertension was significantly lower than that in healthy people,and HDL-C decreased with the increase of blood pressure.HDL-C is negatively correlated with cystatin C and baPWV.Cystatin C and baPWV were significantly increased in patients with hypertension,and increased with the increase of blood pressure.The detection of serum HDL-C,cystatin C and baPWV has auxiliary diagnostic significance in the grading diagnosis of essential hypertension and the judgment of target organ damage,which provides important help for the treatment management and prognosis assessment of patients with essential hypertension.
