Objective: In this article,we will through detecting expression levels of CCL19 and CCL20 in the serum,joint fluid,and synovial tissue of RA patients to analyze their correlation with various clinical and laboratory indicators of RA,and whether they have the diagnosis of RA.In order to provide some new ideas for the diagnosis and treatment of RA.Methods: A total of 138 cases of serum(48 cases of RA,50 cases of OA,40 cases of healthy control(HC)),22 cases of joint fluid(11 cases of RA,11 cases of OA),18 synovial tissues(6 cases of RA,6 cases of OA,and 6 cases of normal control(NC))of RA patients,Osteoarthritis(OA)patients,and normal controls who were treated in the Second Hospital of Lan Zhou University from September 2018 to September 2019 were collected.The concentration of chemokines CCL19 and CCL20 in serum and joint fluid was detected by enzyme linked immunosorbent assay(ELISA).The expression levels of chemokines CCL19 and CCL20 in synovial tissue were detected by immunohistochemistry.Spearman correlation analysis was used to analyze the correlation between the clinical and laboratory indicators of RA and serum chemokines CCL19 and CCL20.Receiver operating characteristic curve(ROC)was used to analyze the diagnostic value of chemokines CCL19 and CCL20 as diagnostic indicators of rheumatoid arthritis.Results: 1.In serum,the concentration of chemokines CCL19 and CCL20 in RA group were significantly higher than those in OA group and healthy control group(Healthy Control,HC)(P<0.01);in joint fluid,The concentration of CCL19 and CCL20 in RA group was higher than that in OA group(P<0.05);in synovial tissue,the expression of CCL19 and CCL20 were visible under microscope in RA group.2.The correlation analysis of the concentration of chemokine CCL19 in the serum of RA patients and the clinical and laboratory indicators of RA shows that the level of chemokine CCL19 in the serum of RA patie are positively correlated with rheumatoid factor(RF),anti-cyclic Cyclic Citrullinated Peptide(CCP)antibody,C-reactive protein(CRP),RA disease activity(DAS28),28 tender joint counts(28TJC),28 swelling joint counts(28SJC)and not related to erythrocyte sedimentation rate(ESR)and age.3.According to the ROC curve,the area under the ROC curve of the chemokine CCL19 is 0.86,the sensitivity of the diagnosis is 68%,and the specificity is 92%.The Youden index is 0.6125,the cut off value is 493.6pg/ml,and the 95% confidence interval is 0.76-0.92.This indicates that the chemokine CCL19 has moderate diagnostic value for RA.4.Correlation analysis of the concentration of chemokine CCL20 in serum of RA patients and various clinical and laboratory indicators of RA shows that the concentration of chemokine CCL20 in serum is correlated with anti-CCP antibody and CRP,and are not related with RF,ESR,Age,DAS28,28 TJC,28SJC.5.According to the ROC curve,the area under the ROC curve of the chemokine CCL20 is 0.95,the sensitivity of the diagnosis is 89%,and the specificity is 92%.The Youden index is 0.82,the cut off value is 193.8 pg/ml,and the 95% confidence interval is 0.886-0.987.This indicates that the expression level of serum chemokine CCL20 may have a high diagnostic value for RA,but the correlation between the clinical and laboratory indicators of combined serum chemokine CCL20 and RA is low,and this conclusion needs to be further confirmed.Conclusion: 1.The expression levels of chemokines CCL19 and CCL20 are higher in serum,synovial fluid of RA patients,which may be involved in the pathogenesis of RA.2.The chemokine CCL19 has a certain correlation with RA-related clinical and laboratory indicators and has high sensitivity and specificity.It has certain clinical diagnostic value for RA.
