| Objectives:Granulomatous mastitis(GM)is a chronic nonspecific inflammatory lesion of the breast,the cause is unknown,and the treatment is currently quite difficult.The Thunder Acupuncture Comprehensive Treatment Instrument is a professional breast disease treatment device.In this study,the treatment device was combined with Mammotome minimally invasive rotary excision+irrigation-drainage+oral prednisone acetate tablets to treat granulomatous mastitis.The purpose of the study is to expand the therapy ways in granulomatous mastitisMethods:Retrospectively analyzed the patients whose pathological diagnosed as granulomatous mastitis in the Breast Department of the First People's Hospital of Kunming City from September 2017 to December 2019.They were dividing into a control group and an experimental group according to the treatment method,the control group used Mammotome minimally invasive rotary excision+irrigation-drainage+oral prednisone acetate tablets(Sinopharm Group Rongsheng Pharmaceutical Co.,Ltd.,5mg/tablet);the experimental group used Mammotome minimally invasive rotary excision+irrigation-drainage+oral postoperative Nisone acetate tablets(same as above)+The Thunder Acupuncture comprehensive treatment instrument(Shenzhen Guoji GK-2200B).Inclusion criteria:(1).Excluding breast tuberculosis and specific granulomatous lesions,patients with pathological diagnosis of granulomatous mastitis;(2).The longest diameter of the tumor measured by ultrasound is within 4-8cm,with obvious Liquefied;(3)No bacteria growth in secretion culture;(4).Infection index is within the normal range,without systemic symptoms;(5).Serum prolactin levels are normal;(6).No contraceptives or psychotropic drugs Taking history;(7).Female patients aged between 20-45 years old;(8).Patients with no nipple depression and only unilateral breast.Exclusion criteria:(1).Inflammatory lesions have caused breast sinus tract,or skin ulceration;(2).Intolerance to hyperthermia;(3).Pregnancy patients;(4).Having metal implants in the body;(5).Those who did not follow up regularly and the data were seriously incomplete and affected the judgment of efficacy;(6).Those who could not insist on treatment and withdrew midway.The total number of cases collected:34 cases in the control group and 44 cases in the experimental group.The cut-off point of observing time was 2 months after oral predisone acetate tablets(or combined with Thunder Acupuncture comprehensive treatment instrument),analyze the changes of the degree of breast pain,the range of skin redness and swelling,and the longest diameter of the mass in the two groups were analyzed under the premise of the same treatment time,and the therapeutic effect of the two groups was comprehensively evaluated.And the occurrence of side effects/adverse events.Results:There were no statistical differences in the age,marriage and childbirth history,location of the disease,side effects,adverse events of the two groups(p>0.05).Before treatment:The pain value before treatment in the experimental group was 5.82±1.32,and the control group was 6.03± 1.60.There was no difference in the degree of pain before treatment in the two groups(p=0.53),no difference in the range of breast redness and swelling(z=0.83),and the longest diameter of the tumor(p=0.38),That is to say,the cases in the experimental group and the control group had the same severity before treatment.After treatment:the total effective rate in the experimental group was 97.7%and 2.3%was ineffective;the total effective rate in the control group was 88.2%and 11.8%was ineffective.There was no significant difference in the total effective rate between the 2 groups(p=0.09),but the cure rate in the experimental group(31.8%)was significantly higher than the control group(14.7%).Among the various grading indicators,the reduction in pain value,the longest diameter of the mass and the reduction in the range of redness and swelling were greater in the 2 groups of patients before and after treatment,and the reduction in pain value and the reduction in the area of redness were greater in the experimental group.The control group was more obvious and there was a significant difference;in the change of the longest diameter of the tumor,the cases of both groups were significantly reduced,but there was no difference between the groups(p=0.529).Conclusions:The treatment characteristics of Thunder Acupuncture comprehensive treatment instrument combined with Mammotome minimally invasive rotary excision in GM are summarized as follows:1.The advantages of Thunder Acupuncture comprehensive treatment instrument in centralized medical diagnosis and treatment,under the conditions of green non-invasive,no radiation,no pain,no sequelae,convenient and easy to operate,with point acupuncture,electrical stimulation,electroporation,bionic massage,and other technologies,precise delivery is achieved through meridian dredging to reach the lesion,which has the effect of softening and loosening,anti-inflammatory and analgesic,clearing heat and detoxification,and promoting metabolism.Give patients a sense of comfort and a relatively high quality of life.2.The Thunder Acupuncture comprehensive treatment instrument is to treat both breasts at the same time,on the one hand,it can improve the condition of the affected breast,on the other hand,it has the health care and shaping effect on the healthy breast.3.With the Mammotome minimally invasive rotary excision cutting method,accurate resection can be achieved through positioning,which has the characteristics of small operative opening,short operation time,small damage range,postoperative dressing change and fewer times of dressing change,which fully guarantees the appearance of the affected breast and double Milk symmetry,easy to be accepted by patients.The use of Mammotome circumcision not only avoids the damage to the breast caused by the repeated operation of hollow needle biopsy,but also can suck pus and open tunnels for catheter drainage,and the amount of specimens can meet the needs of histopathology.The minimally invasive biopsy channel generally chooses to enter from the posterior breast space to the underside of the lesion,which minimizes the damage to the mammary gland lobule and catheter,so as to better preserve the breast feeding function.4.The combination of Thunder Acupuncture comprehensive treatment instrument combined with Mammotome minimally invasive rotary excision+irrigation-drainage+oral prednisone acetate tablets has the trend of controlling breast inflammation,relieving pain,eliminating skin redness and swelling,and improving the cure rate. |
