Research background:Granulomatous mastitis,also known as non-lactating granulomatous lobular mastitis,is a chronic inflammatory disease occurring in the breast.Although compared with other benign breast diseases,the incidence rate is lower and it is not common in clinic,some reports show that more and more patients suffer from this disease in recent years.Moreover,because the diagnostic criteria of granulomatous lobular mastitis are not clear,the treatment scheme is not uniform,and the causes of its occurrence and development are still under study.The clinical manifestation and auxiliary examination are lack of specificity,so the whole treatment system of this disease is confused in clinic,and misdiagnosis and mistreatment often occur,which brings great pain to patients.Therefore,it is extremely urgent to increase the research on GM and improve the understanding of the disease among clinicians.GM mostly occurs in young and middle-aged women after delivery,and the children are mostly under five years old when the disease occurs.at present,the popular views on the causes of the disease include smoking,breast trauma,birth control pills,diet pills,taking history of psychotropic drugs,nipple invagination,etc.the clinical diagnosis and treatment measures for the disease include steroid treatment,traditional Chinese medicine treatment,surgical treatment,etc.This article takes the sex hormone level as the breakthrough point,and explores the understanding of the occurrence and development of the disease from the sex hormone level,in order to seek new ideas and treatment plans for the diagnosis and treatment,and bring good news to the majority of GM patients.Research purposes:In this study,the sex hormone levels of patients with granulomatous mastitis were detected to explore whether the sex hormone levels are related to the occurrence and development of GM.In addition,the patients with granulomatous mastitis with obviously increased prolactin level were treated by integrated traditional Chinese and western medicine,so as to further explore the influence of auxiliary correction of prolactin level on the treatment and prognosis of granulomatous mastitis.The purpose of this study is to explore a treatment scheme that can shorten the clinical cure time and avoid postoperative recurrence,and provide a reliable basis for clinical diagnosis and treatment of patients with granulomatous mastitis.Research methods:The first part:1.Choose to visit the breast surgery clinic of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine.According to the clinical manifestations and imaging examination,the female patients diagnosed as suspected GM were examined for six items of female hormones in their early follicular phase.Record data.After surgical treatment,80 patients with granulomatous lobular mastitis were randomly selected.Register as the experimental group.Another 30 normal women were selected as control group.2.The six hormone levels of the two groups were statistically analyzed one by one,to explore whether the differences were statistically significant,and according to the results,to explore which hormone level or hormone levels were related to GM.3.80 patients in the observation group were staged according to different clinical symptoms,and the six hormone levels were compared one by one.According to the results,it was discussed which hormone level or hormone levels were related to the clinical stages of GM,or there was no clear relationship.Part two:1.Fifty-six patients with elevated serum prolactin in the breast surgery clinic of the First Affiliated Hospital of Tianjin University of Traditional Chinese Medicine from March 2019 to September 2020,who met the inclusion and exclusion criteria,were divided into treatment group and control group according to whether to use prolactin-lowering drugs.The treatment group was treated with traditional Chinese and Western medicine for inhibiting prolactin.The control group was treated with traditional Chinese and Western medicine.28 cases in each group.2.By studying the difference of postoperative recovery effect,recurrence rate,recurrence time and breast shape evaluation between the treatment group and the control group,the influence of adjuvant correction of prolactin level on the treatment and prognosis of granulomatous mastitis was explored.Research results:The prolactin level in the observation group was higher than that in the control group,and the difference was statistically significant.The levels of estrogen,progesterone,testosterone,luteinizing hormone and follicle stimulating hormone in the observation group were not significantly different from those in the control group.Statistical analysis was carried out between the levels of sex hormones in different periods and between them,and no significant difference was found.Conclusion:1.The serum prolactin level of 1.GM patients has a certain correlation with the occurrence of granulomatous mastitis,but the specific correlation mechanism is not clear.2.Serum progesterone,estrogen,luteinizing hormone,follicle stimulating hormone,testosterone levels are not related to the onset of granulomatous mastitis.3.The levels of sex hormones among the various pathological stages of granulomatous mastitis are equivalent,and there is no correlation between the serum sex hormone levels of GM and the pathological stages.4.Traditional treatment of integrated traditional Chinese and western medicine has a certain clinical effect on GM patients.5.Correction of prolactin level combined with traditional Chinese and Western medicine treatment is relatively better than traditional Chinese and Western medicine treatment alone.6.Correction of prolactin level combined with traditional Chinese and Western medicine treatment can shorten the clinical cure time,reduce the recurrence rate and improve the satisfaction of postoperative breast shape.
