Objectives:In recent years,there has been increasing research on the use of minimally invasive methods of corticotomy-assisted orthodontic treatment(CAOT),and many studies have confirmed that minimally invasive methods are as effective as traditional corticotomy.This topic combines 3D surgical guide with minimally invasive corticotomy to measure,analyze and compare the differences between 3D surgical guide combined with minimally invasive corticotomy and traditional corticotomy in terms of health index of periodontal and dental tissue,quality of life,tissue healing and scar formation,etc.Which will provide reference for 3D guide research in the future.Methods:Clinic in our hospital's patients with orthodontic division into standard were collected and divided into tradition group and 3D group,3D group line 3D plate combined with minimally invasive corticotomy,the tradition line of traditional corticotomy(single flap corticotomy).Clinical observation indicators include:1.Periodontal health index,including preoperative and postoperative 6-week,12-week and 24-week probing pocket depth(PPD)and modified gingival index(mGI).2.Pulp health status,including pulp activity before and 24 weeks after operation.3.The duration of surgery and the extent of the impact of surgery on oral health were recorded in Oral Health Impact Profile(OHIP-14)and the impact of oral health on the quality of life was evaluated before 3 days after surgery and 7 days after surgery.4.Periodontal aesthetic index,periodontal oral mucosal scar was evaluated by three different physicians(periodontal,orthodontic,restoration)after 24 weeks of surgery.Sorting out the data of each stage for statistical analysis and getting the results.Results:1.Between the two groups of patients,no statistical differences in periodontal health index(PPD)appeared before operation or 18 weeks,24 weeks respectively after operation(P>0.05),but the statistical differences appeared 6 weeks after operation(P<0.05).And no statistical differences in mGI occurred before operation or 6 weeks,18weeks respectively after operation(P>0.05),but the diffrences that occurred 24 weeks after operation were statistically significant(P<0.05).There was no significant difference in periodontal health index PPD and mGI between the two groups compared with baseline in each observation period(P>0.05).2.There was no statistically significant difference between the measured teeth and the control teeth before and after the dental pulp electric vitality test(P>0.05).3.(1)The duration of operation in the 3D group was shorter than that in the tradition group,and the difference was statistically significant(P<0.05);(2)The oral health quality of life of patients in the 3D group was significantly lower than that in the tradition group in three aspects:swelling and pain,eating,and daily life(3 days and 7 days after surgery)(P<0.05)and there was no difference in the total quality of life score between the two groups before operation(P>0.05),after 3 days,7 days were significantly lower than that of tradition group(P<0.05).The total score of oral health quality of life of patients in the 3D group was significantly higher than that at baseline 3 days after surgery(P<0.05)but not significantly different from baseline at 7 days after surgery(P>0.05).The scores of patients in the tradition group were significantly higher than the baseline at 3 and 7 days after surgery(P<0.05).4.The scores of mucosal scars at 24 weeks after operation in the three groups of doctors were higher in the tradition group than in the 3D group(P<0.05)and there was no difference among the scores of the three groups by physicians(P>0.05).Conclusions:The 3D surgical guide can be used for minimally invasive corticotomy.Compared with traditional corticotomy,it can shorten procedure times efficiently,reduce the postoperative pain and discomfort of patients,enhance the patients' qualities of life after surgery,with high patient acceptance and better aesthetic effect of the operation area.
