Objective:To study the risk factors of preterm birth,the common complications of premature infants and their prognosis,in order to provide theoretical basis for reducing the rate of preterm birth and the incidence of complications of premature infants,and improving the clinical outcome of premature infants,and improving the quality of life of premature infants.Materials and Methods:Part ?:Collected general clinical data of some women who gave birth to live babies in the Third People's Hospital of Yancheng during the period from January 1,2016 to December 31,2018.Among them,359 premature mothers were set to the premature delivery group and the other 6829 cases of normal full-term laborers were set to full-term group.All the data were analyzed retrospectively with SPSS 20.0 software.The count data were expressed by the composition ratio or rate.Chi-square test was used for inter-group comparison.Logistic regression was used to analyze the influencing factors,according to the inspection level,p<0.05 showed statistically significant difference.Aim to discuss the high-risk factoes of preterm birth.Part ?:Collected 495 cases of premature infants hospitalized in NICU of the Third People's Hospital of Yancheng from January 1,2016 to December 31,2018.Collected the informations included gender,gestational age,birth weight,onset time and hospitalization days,and postnatal diseases of premature infants,like neonatal asphyxia,neonatal respiratory distress syndrome,apnea,bronchopulmonary dysplasia,pulmonary hemorrhage,intracranial hemorrhage,neonatal hypoxic ischemia encephalopathy,purulent meningitis,neonatal pneumonia,neonatal anemia,necrotizing enterocolitis,neonatal hyperbilirubine-mia,neonatal hypoglycemia,neonatal congenital malformation,neonatal respiratory failure,etc.The classification of diseases was based on the international disease classification method(IDC-10).The names of the diseases were referred to‘Practical Neonatology',and the prognosis of the patients was calculated.Preterm infants were classified according to different classification methods.SPSS 20.0 software was used for statistical analysis of the data.The counting data were expressed by composition ratio or rate.Chi-square test was used for inter-group comparison,p<0.05 showed statistically significant.Multivariate logistic regression analysis was used for the analysis of risk factors between groups,and univariate logistic regression was used for the analysis of complications between groupsResults:1.Collect and statistically analyzed the information of age,the number of previous abortion,pregnancy complications,whether artificial assisted pregnancy and single or twin(multiple)fetuses in the preterm and full-term groups.Among the data collected in this study,only single pregnancy and twin pregnancy were found.Therefore,this study only analyzed the influence of twin pregnancy on premature birth.The results showed that previous abortion,scar uterus,preeclampsia,intrahepatic cholestasis of pregnancy,fetal abnormalities,fetal intrauterine distress,premature rupture of membranes,twin pregnancy,and placental abnormalities were risk factors for premature birth,and all of them were independent factors(OR>1).2.There were statistically significant differences in the birth weight of premature infants of different gestational ages(p<0.05).The smaller the gestational age,the higher proportion of low birth weight infants.3.With the increase of gestational age and birth weight,the incidence of neonatal asphyxia,neonatal respiratory distress syndrome,apnea,bronchopulmonary dysplasia,brain injury and neonatal anemia decreases gradually.4.Gestational age,birth weight and neonatal asphyxia,neonatal respiratory distress syndrome,bronchopulmonary dysplasia,infectious diseases,neonatal anemia,neonatal hyperbilirubinemia and other complications are independent factors influencing the length of hospitalization.5.Among the many risk factors that affect the clinical prognosis of premature infants,gestational age,weight and complications play a key role.Conclusion:1.Many factors may lead to premature birth in the course of maternal pregnancy,among which previous abortion,scar uterus,preeclampsia,intrahepatic cholestasis of pregnancy,fetal abnormality,fetal intrauterine distress,premature rupture of membranes,twin pregnancy and placental abnormality are risk factors leading to premature birth,and all of them are independent influencing factors.2.The main factors affecting the prognosis of premature infants are:gestational age,birth weight and complications,which usually interact with each other and jointly determine the outcome of premature infants.
