BackgroundThe standardized training of resident doctors is a transitional stage for all medical students from medical students to formal staff,and a bridge between medical colleges and medical institutions.The standardized training of resident doctors is an important node in the process of medical talent development.However,due to the work pressure and psychological gap faced by resident doctors in this process,it is easy to cause the resignation of resident doctors,and even lead to mental illness,resulting in irreversible consequences.Up to now,the perceived stress and the intention to stay in the post have aroused wide concern in the society.How to give them a good working environment,reduce the working pressure of the doctor;and give the correct career guidance,prevention and control regulations with the end of the doctor impulse resignation,resulting in brain drain phenomenon.Therefore,it is significant to find correlation and influencing factors between perceived stress and job retention among resident doctorsObjectivesThe purpose of this study was to(1)explore the influencing factors and correlation between the perceived stress and the job retention in Shandong province,and the influencing factors.(2)By investigating the current situation of the perceived stress and the job retention in Shandong province,the author analyzed the deep-seated reasons and their correlation that affect the perceived stress and the job retention in Shandong province.(3)In view of the problems existing in the perceived stress and the job retention on the post,this paper puts forward some suggestions for the relief of the pressure on the work of the doctors in the national standard training and the correction of career guidance,and provides a reference for the improvement of the relevant policies of the training system of the standard training program in China.MethodsIn this study,cross sectional study was used to conduct an electronic questionnaire survey on the resident doctors in 9 large grade comprehensive hospitals in Shandong province by means of cluster sampling.The electronic questionnaire used the Perceived Stress Scale,the Job Retention Scale and the self-designed scale to investigate the perceived stress,job retention and the socio-demographic characteristics of the resident doctors respectively.The pressure perception scale score is 25 and above,which is judged as a health risk stress;the job retention scale score is in the range of 1 and 3.66,which is judged as low and middle job retention.A total of 3,528 effective questionnaires were collected,with a recovery rate of 78.40%,including those from obstetrics,gynecology,dentistry,pediatrics,radiology and other departments.The data were analyzed by SPSS 24.0 statistical software.The continuous variables are expressed as mean±standard deviation.Rate and composition ratio representation of counter data.The chi-square test was used to compare the distribution of two or more groups of discrete variables.Univariate regression and correlation analysis were used to explore the main influencing factors of perceived stress and job retention on the job in physicians,the relationship between perceived stress and job retention on the job and multiple factors was analyzed by binary logistic regression,the correlation between perceived stress and intention to stay was analyzed by R correlation analysis.Results and DiscussionThe mean age of the included sample was 29.38±2.60 years.The percentage of women was slightly higher,accounting for 59.7 percent of the total.In the discipline of planning and training,the number of doctors of internal medicine planning and training is the largest,accounting for 27.6%of the total number of doctors.The number of doctors with the income of 3,001-5,000 yuan is the largest,accounting for 35.2%of the total population.In this study,the perceived stress of the sample was 25.20±7.82,and 52.6%of the resident doctors were under health risk stress.The job retention score of the sample was 3.56±0.74,and 58.4%of the resident doctors were at a low level.Chi-square test results showed that gender,student identity,household registration,resident doctor profession,monthly average income,rotation department distribution performance and average number of management beds all had statistically significant differences in the bicategorization ratio of stress perception score,indicating that this factor would affect the perceived stress of resident doctors.Besides,there were statistically significant differences among gender,student identity,training time at the base,whether resident doctors had obtained physician qualification certificate,discipline planning,average monthly income,rotation department distribution performance,and average number of management beds,indicating that these factors would affect the job retention of resident doctors.The collinearity diagnosis showed that the mean monthly income and rotation department distribution performance issued variance inflation factor value is greater than 10,two factors exist serious collinearity.Therefore,rotation department distribution performance was eliminated,gender,household registration,the student identity,resident doctor profession,average income,average monthly management bed as influencing factors included in the binary logistic regression model.And gender identity,all the students,whether doctors as certificate,cultivating professional rules,monthly average revenue,average management data,enter the base training time,perceived stress level as influencing factors included in the binary logistic regression model.There were statistically significant differences in the influence of the all included variables on perceived stress and job retention.R correlation analysis showed that the two dimensions of stress perception(tension and sense of loss of control)were negatively correlated with total score of job retention on the job.The higher the degree of tension or loss of control,the lower the job retention was.Conclusion and SuggestionsThrough quantitative research design,this study analyzed the correlation between the perceived stress and the job retention in Shandong province,and the main conclusions were as follows:(1)More than 50%of the resident doctors' overall perceived stress was in danger pressure.(2)More than 50%of the resident doctors' job retention o is at a medium or low level.(3)The difference of the discipline will affect the stress perception and job retention of the resident doctors.(4)The stress perception level and retention intention of regular training students are related to individual salary and treatment.The higher the salary is,the lower perceived stress is,and the higher job intention is.Based on this study,the paper puts forward the following Suggestions:(1)improve treatment,and the management of performance appraisal,perfecting the incentive mechanism,safeguard rules with students rights(2)actively carry out rules with student ideological education work,to establish complaint mechanisms,understand their professional pressure,channel rules with students emotion(3)help resident doctors with career planning,and the relationship between the life and work balance(4)to establish conforms to the social needs and regular medical talent training mode,relieve
