BackgroundAs the most common benign tumors in the uterus,endometrial polyps often cause abnormal uterine bleeding and infertility.Its etiology is unknown,most previous opinions considered this to be a hormone-dependent disease.But more and more studies have found that the presence of micropolyps under hysteroscopy may be associated with chronic endometritis.Therefore,some scholars have suggested that the occurrence of some endometrial polyps is associated with inflammation,that is,chronic endometritis may promote the occurrence of polyps,and endometrial polyps can represent the local changes in the endometrium caused by chronic inflammation However,at present,there is a lack of research on endometrial polyps with chronic endometritis and pregnancy outcome.Therefore,the purpose of this study is to conduct a retrospective study of patients diagnosed with endometrial polyps,combined with clinical follow-up results,to explore the correlation between endometrial polyps and chronic endometritis and its impact on women of reproductive age.Objectives1.To investigate the effect of endometrial polyps combined with chronic endometritis on assisted reproductive outcomes in infertile women2.To explore the prevalence of endometrial polyps with chronic endometritis..3.To explore whether there is a difference in the proportion of patients with chronic endometritis among those with or without infertility4.To explore the relationship between infertility type and endometrial polyp with chronic endometritis.5.To explore the related high-risk factors of endometrial polyps with chronic endometritis,and to provide clinical basis for the prevention and diagnosis of endometrial polyps.Methods1.A total of 370 women of reproductive age who underwent hysteroscopy who were admitted to Shenzhen Maternal and Child Health Hospital from 01 January 2016 to 31 December 2017 were selected as research objects.They were divided into polyps and non-polyp groups according to hysteroscopy and pathology,polyp group was divided into infertility group and non-infertility group according to medical history,and retrospective analysis of general clinical data and hysteroscopy performance of all subjects2.Collect endometrial tissue samples for Hematoxylin and eosin staining and CD138 immunohistochemical staining respectively.3.Telephone or outpatient follow-up assisted reproductive pregnancy outcomes for women with fertility requirements in the polyps group 2 years after hysteroscopy surgery.Results1.Follow-up of assisted reproductive outcomes for patients with polyps and infertility 2 years after hysteroscopy found that the clinical pregnancy rate,embryo implantation rate and live birth rate of patients with polyps and chronic endometritis were significantly lower than those of patients with simple polyps,and the risk of early miscarriage increases,and the difference in biochemical pregnancy abortion was not statistically significant.2.CD138 positive rate of endometrial polyps among women of reproductive age was 24.68%,of which 39.2%in the infertility group,which was significantly higher than that in the fertility group.3.Among people with polyps and infertility,the proportion of endometrial with cd138 positive in was 28.60%in the primary infertility group and 52.70%in the secondary infertility group,which differences are statistically significant4.History of inter-menstrual bleeding,miscarriage and infertility,especially the history of secondary infertility is a high risk factor for endometrial polyps with chronic endometritis.Conclusions1.Endometrial polyps are associated with chronic endometritis and are more common in patients with infertility,especially secondary infertility.Therefore,the diagnosis and treatment of chronic endometriitis should be emphasized.2.Endometrial polyps with chronic endometritis can affect the assisted reproductive outcomes of infertile patients3.For patients with inter-menstrual bleeding,miscarriage,and infertility,especially those with secondary infertility may be the high risk factor for patients with endometrial polyps and chronic endometritis,it is recommended to further immunize the pathology with CD138 Immunohistochemistry staining for clear diagnosis.
