Background: China has become the country with the largest elderly population in the world,and has stepped into an aging society.Subsequently,senile diseases including senile dementia,senile psychosis and senile vascular sclerosis gradually increase,bringing heavy burden to society,medical treatment and family.Vascular dementia is the second most common type of dementia after alzheimer's disease.Related studies have shown that neurobiological changes after brain tissue damage are closely related to the psychiatric symptoms of vascular dementia.In patients with vascular dementia accompanied by psychiatric behavioral symptoms,there is a decrease in cholinergic cells in specific areas of the brain,and donepezil can improve the level of acetylcholine between cells synapses and improve the psychiatric behavioral symptoms of patients.Studies have confirmed that donepezil alone cannot achieve ideal therapeutic effects.Antipsychotics are often used off-label,although drug treatments for dementia-related psychosis have not yet been approved.At present,many scholars are committed to the use of quetiapine to treat mental disorders in patients with dementia,believing that quetiapine can not only improve mental disorders,but also improve patients' cognitive disorders.Objective: This study is dedicated to the analysis of the efficacy of donepezil combined with quetiapine in the treatment of cognitive function and mental behavior abnormalities in patients with vascular dementia,To provide a reference for clinical medication guidance for patients with vascular dementia.Methods: The subjects of this experiment were patients with vascular dementia who were hospitalized in the Department of Geriatric Neurology at the First Affiliated Hospital of Henan University from March 2018 to September 2019.When the condition is stable within 14 days after admission,they are divided into experimental group and control group according to the random number table and random number grouping method,with 40 people in each group.The experimental group was treated with donepezil combined with quetiapine,The control group was given only donepezil.Patients were evaluated for cognitive function,psycho-behavioral symptoms,and daily living ability before and 3 months after treatment.The general data and past history of patients were collected,including age,gender,education level and the composition of common chronic diseases such as hypertension,diabetes,and hyperlipidemia.All the above data were statistically analyzed using SPSS24.0 software.Results:The 80 patients eligible for inclusion in the study,including 40 in the experimental group and 40 in the control group.Before treatment,the scores of the MMSE scale of the two groups of patients were reduced to varying degrees,and the scores of the ADL and NPI scales were increased to varying degrees.There was no significant difference in the scales of the scores between the two groups of patients(P> 0.05);After 3 months of treatment,the scores of the MMSE scale increased,and the scores of the ADL and NPI scales decreased.The differences between the two groups were statistically significant(P <0.05);The score of the MMSE scale of the experimental group was higher than that of the control group,and the difference was statistically significant(P <0.05),but the scores of the ADL and NPI scales were not significantly different(P> 0.05);The experimental group had a higher incidence of adverse reactions than the control group,but the difference was not statistically significant(P <0.05).Conclusion:Donepezil combined with quetiapine and donepezil alone can improve cognitive function,daily living ability,and psycho-behavioral symptoms in patients with vascular dementia;donepezil combined with quetiapine is more effective than donepezil alone,and there is no significant difference in the incidence of adverse reactions between the two treatments.
