| Objective: This study aims to explore the therapeutic effect of traditional Chinese medicine(TCM)on vascular dementia from the clinical aspect by observing the therapeutic effect of donepezil,a cholinesterase inhibitor,on vascular dementia.Methods: from January 2019 to January 2020,99 patients with vascular dementia met the inclusion criteria in the outpatient department of Neurology Department of Ruikang Hospital Affiliated to Guangxi University of traditional Chinese medicine.According to the method of random number table,the selected patients with vascular dementia were divided into control group A(34 cases),control group B(32 cases)and treatment group(33 cases).During the observation,9 patients were excluded due to the reasons of loss of visit,receiving other treatment and not following the required treatment In addition to the cases,the final case control group a 31 cases,control group B29 cases and treatment group 30 cases.All the three groups were given the treatment for the primary disease,such as hypertension,diabetes,cerebral infarction and other diseases to control blood pressure,blood sugar,antiplatelet aggregation,lipid regulation and plaque stabilization,and other drugs unrelated to the primary disease that would interfere with the results should not be used,such as huperzine A tablets.The control group A wasgiven 5 mg donepezil hydrochloride tablets once a day.The control group B was treated with the method of Tonifying the kidney and strengthening the spleen as the basic treatment,and was given the decoction of Tonifying the kidney and strengthening the spleen and supplementing the Qi(wolfberry fruit 30 g,Ophiopogon japonicus 15 g,Poria cocos 9g,Atractylodes macrocephala 9g,Chenpi 9g,ginseng 6g,Shengdi 6g,)recorded in Volume II of the "wide notes of xianxingzhai medicine".According to the syndrome,one dose per day,decoction 200 ml in water,twice a day in the morning and evening.In the treatment group,donepezil hydrochloride(5mg,once a day,oral)+ Bushen Jianpi Yiqi Decoction(one dose per day,water decoction200 ml,twice a day in the morning and evening)was given at the same time.The course of treatment was 90 days.After the treatment,the clinical effects of the three groups were observed,including the score of cognitive function(Mo CA),ADL(Barthel Index)and SDSVD before and after the treatment.Results: 1.Cognitive function(Mo CA)score: after treatment,MOCA score of the three groups was higher than before treatment,the difference was statistically significant(P < 0.05).Compared with the control group A,there was no significant difference in the integral of the control group B(P > 0.05).There was significant difference between the treatment group and the control group(P < 0.05).It is suggested that the cognitive function of the three groups is improved after treatment,and the improvement of cognitive function in the treatment group is better than that in the control group A and B.2.ADL(Barthel Index)scores: after treatment,ADL scores of three groups were higher than before treatment,the difference was statistically significant(P < 0.05).Compared with control group A,there was no significant difference in ADL score of control group B(P > 0.05).There was significantdifference between the treatment group and the control group(P < 0.05).Conclusion: after treatment,the activities of daily living in the three groups were improved,and the improvement of activities of daily living in the treatment group was better than that in the control group A and B.3.TCM syndrome score(SDSVD): before treatment,there was no significant difference in TCM syndrome score among the three groups(P > 0.05);after treatment,the TCM syndrome score of the three groups was lower than before treatment,the difference was statistically significant(P < 0.05).Compared with the control group A,there was no significant difference in the scores of the control group B(P > 0.05).Compared with control group A and control group B,the scores in the treatment group were statistically significant(P <0.05).Conclusion: after treatment,the TCM Syndromes of the three groups were improved,and the improvement of TCM syndromes in the treatment group was better than that in the control group A and B.Conclusion: 1.The simple treatment of anlishen,Bushen Jianpi Yiqi Decoction,and Bushen Jianpi Yiqi Decoction Combined with anlishen have curative effect on vascular dementia,which can improve the clinical symptoms and cognitive function of vascular dementia patients in different degrees,and improve the ability of daily life and activity of patients.2.The therapeutic effect of Bushen Jianpi Yiqi Decoction Combined with anlishen on vascular dementia is the best,which is worthy of clinical application. |
