ObjectiveTo investigate the clinical significance of predicting chronic complications of type 2 diabetes(T2DM)by inflammatory indicators such as neutrophil-lymphocyte ratio(NLR),platelet-lymphocyte ratio(PLR),white blood cells,C-reactive protein(CRP),and erythrocyte sedimentation.Methods:A randomized cohort was selected from 2017.1 ? 2019.6 125 cases of T2 DM were newly diagnosed in the Endocrine Diabetes Department of the First Affiliated Hospital of Henan University(the diagnosis was based on WHO(1999)diagnostic criteria for diabetes).Analysis of the association between neutrophil-lymphocyte ratio(NLR)and complications of T2 DM.Thirty-two patients with T2 DM in the Department of Endocrinology and Diabetes,the First Affiliated Hospital of Henan University,were selected as the control cohort study from 2017.1 to 2020.1.The general data and blood routine,C-reactive protein(CRP),erythrocyte sedimentation,blood glucose,and glycated hemoglobin content of the patients were recorded,and the NLR and PLR values were calculated.The NLR and PLR values of each group were compared and analyzed.Results:Among the patients with T2 DM complications in the randomized cohort,42 were small vessel disease and 26 cases were macrovascular disease.24 cases of large and small vessel disease.Among the patients with small vascular complications,8 cases had diabetic nephropathy,5 cases had diabetic retinopathy,7 cases had diabetic peripheral neuropathy,and 22 cases had multiple small vascular complications.For macrovascular complications,4 cases had diabetic cardiovascular disease,4 cases had diabetic cerebrovascular disease,7 cases had diabetic arterial vascular disease,and 11 cases had multiple macrovascular complications.Among all diabetic patients,the proportion of T2 DM complications(undiagnosed T2 DM patients)at the time of initial diagnosis is 73.6%.There was no statistically significant difference in age,white blood cells,ESR,smoking,and alcohol consumption between patients with diabetes complications and those without complications,and the differences in gender,BMI,blood glucose,glycated hemoglobin,NLR,PLR,and CRP The difference was statistically significant.Logistic regression analysis of factors related to thrombotic events that may be complications of diabetes: age,BMI,smoking,alcohol consumption,blood glucose,glycated hemoglobin,NLR,and PLR.The results showed that BMI,glycated hemoglobin,NLR,and PLR could be used as indicators of thrombus prediction.Perform ROC curve analysis on NLR and PLR,and the area under the curve is NLR(0.916)and PLR(0.799).The area under the ROC curve of NLR and PLR was tested by Z test,and Z=3.969,P=0.000<0.05,indicating that NLR has higher test power for the occurrence of T2 DM complications.Youden Index NLR(1.905),PLR(98.03).The results of the validation cohort showed that the differences in BMI,blood glucose,glycosylated hemoglobin,NLR,PLR,and CRP were statistically significant.Logistic regression analysis showed that BMI,glycosylated hemoglobin,NLR,PLR are closely related to T2 DM complications.The area under the curve is NLR(0.911)and PLR(0.759).The area under the ROC curve of NLR and PLR was tested by Z test,and Z=3.977,P=0.000<0.05,indicating that NLR has higher test power for T2 DM complications.Youden Index NLR(1.899),PLR(102.3).Conclusions:1.The common age of T2 DM is between 41-51 years old..2.NLR,PLR,BMI,glycosylated hemoglobin values are closely related to T2 DM complications.3.With the increase of NLR and PLR,the risk of T2 DM complications increases,and NLR has a higher test efficiency for the occurrence of T2 DM complications.
