Objective:To evaluate remodeling of the femoral artery and its correlation with neutrophil to lymphocyte ratio(NLR)in patients of type 2 diabetesmellitus(T2DM)by high-frequency ultrasound.And discuss the clinical value of the research.Methods:88 patients with T2 DM were enrolled from the Second Affiliated Hospital of Dalian Medical University.All the diagnosis of T2 DM were made according to the standards of WHO.Exclusion criteria included hypertension,liver and renal diseases,rheumatic disease,malignant tumor,acute infection,the trauma or surgery within two weeks and recent use of steroid hormones.These patients above mentioned were divided into group B(48cases,the course ?10 years)and group C(40 cases,the course >10 years).36 healthy volunteers were recruited as the control group(group A).There were no significant differences in age,sex ratio between groups A,B and C.The color Doppler ultrasound equipment of GE LOGIQ E9 was used which equipped with high frequency line array probe.The type of the probe is 9L-D and the frequency is 6-9MHZ.The subjects were in supine position and the root of the thigh was exposed.The longitudinal scanning was performed,showing the common femoral artery,the bifurcation and the superficial femoral artery.Then the probe was rotated 90 degrees to perform horizontal scanning to observe the general condition of the plaque and the intimal medial thickness(IMT),total vessel area(TVA),lumen area(LA)were measured and vascular remodeling types were determined.NLR was calculated and obtained from the peripheral blood routine examination.Finally,the correlation between IMT and NLR were analyzed.Results:IMT and NLR of the femoral in group B and C were higher than those in group A(P<0.05).IMT and NLR of the femoral in group C were higher than that in group B(all P<0.05).RI in group B was higher than that of group A(P<0.05).RI in group C was lower than that of group B(P<0.05).The vascular remodeling rates in group B and C were higher than that in group A(all P<0.05),and in group C was higher than that in group B(P<0.05).The positive remodeling rate of group B was higher than that of other two groups,and the difference were significant.The rate of negative remodeling in group C was higher than that of group A and B(all P<0.05).Vessels without remodeling was predominant in group A.The rate of negative remodeling was higher than the rate of positive remodeling in group C.The rate of positive remodeling was higher than the rate of negative remodeling in group B.The relativity analysis shows that IMT was positively correlative with NLR(r=0.56,P<0.05).Conclusion:The combination of application of two-dimensional high-frequency ultrasonography and NLR can comprehensively evaluate the progress of the lower extremity vascular disease of T2 DM patients,providing the objective basis for early clinical diagnosis and treatment.
