Objective: Many scholars at home and abroad in recent years have reported that serum neutrophil and lymphocyte ratio(NLR)can effectively alert early diabetes and chronic microvascular complications,this subject will further explore the correlation between NLR in Xining and early diabetic chronic kidney disease.Methods: 320 randomly selected patients were divided into early diabetic nephropathy group(group DN),type 2 diabetes mellitus group(group T2DM)and control group.The changes of NLR levels between the 3 groups were analyzed and compared.Results: the variance of 1,univariate analysis showed that: there was no significant difference between the three groups of BMI,TCH,LDL,Cr,MC,FINS difference(P < 0.05);DN group in age,course of disease,NLR,WBC,NC,Hb A1 c,FPG,2h PG,IR,Cys and C were significantly higher than T2 DM group,and T2 DM group in the above indexes are also higher than the control group,the difference was statistically significant(P<0.05);group DN,waist circumference,BUN higher than T2 DM group and control group,LC and HDL are lower than the diabetic group and control group,and there was no significant difference in between two groups(P >0.05);DN group and T2 DM group(TG X-=9.179 and X-=17.325,P<0.05,hs-CRP(X)-=921.606 and X-=8.864,P<0.05)higher than that of the control group.2 multivariate Logistic,two classification and regression analysis showed that age,duration of disease,Hb A1 c,IR and NLR are the main risk factors of early diabetic nephropathy,which has a greater risk factor NLR(OR=28.609)3,Pearson or Spearman correlation analysis showed that the age,course of disease,Hb A1 c,IR,NLR and microalbuminuria were positively correlated NLR,which has a greater correlation coefficient(r=0.558).Conclusion: 1,in the Xining area,WBC,NC,age,NLR,Hb A1 c,IR,Cys,C with the progression of type 2 diabetes increased gradually,and LC in patients with diabetic nephropathy may be reduced,the abnormal body state of chronic inflammation and immune system double results.2,age,course of disease,Hb A1 c,IR,NLR and microalbuminuria was positively related to early diabetic nephropathy is a major risk factor,in which NLR has larger correlation coefficients and the risk coefficient,can predict early diabetic nephropathy.Conclusion: In Xining,men are more likely to develop type 2 diabetes and develop early diabetic nephropathy than women,and may be related to lifestyle habits such as smoking and alcohol consumption.NC and NLR are gradually increasing with the progression of type 2 diabetes mellitus,but the decrease in LC in patients with nephropathy may be a double result of chronic inflammatory state and abnormal immune system.Compared with age,course of disease,Hb A1 c and IR,NLR has greater OR and is positively correlated with microalbuminuria,which can predict the occurrence of early diabetic nephropathy.Therefore,the detection of blood routine and calculation of NLR may be a new idea for screening early diabetic nephropathy in outpatient and primary hospitals.
