Objective:Colorectal cancer(CRC)is a common malignant tumor of the digestive tract.Carcinoembryonic Antigen(CEA)and carbohydrate antigen 199(Carbohydrate Antigen 19-9,CA19-9)are commonly used for gastrointestinal tumors.Markers,currently CEA and CA19-9 in clinical CRC patients mainly have a certain reference value for their postoperative recurrence and metastasis,but domestic research on the correlation with Overall Survival(OS)of CRC patients is still in progress Relatively small.This study aimed to explore the correlation between the preoperative serum CEA,CA19-9 expression levels and OS in patients with CRC,and provide clinical reference value for the prognosis of CRC patients undergoing clinical radical surgery.Methods:Retrospective analysis of clinical medical records of 105 patients with CRC admitted to radical surgery in the Second Affiliated Hospital of Kunming Medical University from January 2014 to December 2015,including preoperative serum CEA,CA19-9 expression levels,clinical data,Pathological data,etc.,follow-up telephone follow-up,the deadline for follow-up is December 2019,with the patient's death as the end point of follow-up,to determine the survival status(survival or death)after radical surgery.The correlation between positive CEA and CA19-9 expression levels and clinical and pathological factors was analyzed.Univariate analysis of gender,age,tumor site,tumor differentiation,tumor differentiation,TNM stage,and lymph node metastasis,preoperative serum CEA and CA19-9 levels and OS using survivor function(Kaplan-Meier)and related log-rank test Correlation,and incorporate the index into the COX regression model to analyze the correlation with OS of CRC patients.All data were statistically analyzed using IBM SPSS23.0 software.Results:105 patients with CRC were followed up by telephone.The survival time was 7 to 72 months,and the average survival time was 56 months.56 patients(53.30%)survived and 49 died(46.70%).Preoperative serum CEA was negative in 62 cases(59.05%)and positive in 43 cases(40.95%).The positive expression of CEA was correlated with the patient's TNM stage and lymph node metastasis.P<0.05;it was related to gender,age.tumor location and tumor differentiation No correlation,P>0.05.Preoperative serum CA19-9 was negative in 56 cases(53.33%);positive in 49 cases(46.67%).The positive expression of CA19-9 was correlated with the degree of tumor differentiation of the patient,P<0.05;it was related to TNM stage,gender,age,tumor location There is no correlation between lymph node metastasis and P>0.05.The TNM stage,tumor differentiation,preoperative CEA,CA199 expression levels of the CRC patients and the combined detection of both were related to the OS of the CRC patients,P<0.05;the OS of the CRC patients was related to gender,age,tumor location,and lymph nodes.Positive is not relevant,P>0.05.The preoperative serum CEA and CA19-9 were both negatively expressed.The median overall survival(Median Overall Survival,MOS)was 68 months.Both were positively expressed.MOS was 50 months,and one of the two positive expressions.The MOS was 66 months,and the log-rank test P<0.05,the difference between the three was statistically significant.The MOS of TNM stage ?,?,and ? were 69 months,66 months,and 46 months,respectively.In stage ? and ?,the later the tumor stage was,the shorter the MOS was.The difference between the three is correlated,P<0.05.The low,medium,and high differentiation MOS of the tumors were 43.5 months,61 months,and 69 months,respectively.The high and medium differentiation MOS was significantly prolonged,that is,the better tumor differentiation was positively correlated with the OS of CRC patients.The difference between the three was correlated,P<0.05.The OS of CRC patients with negative CEA expression(MOS 66 months)was significantly longer than that of positive patients(MOS 58 months),HR=3.341,95%CI=1.592?7.01 1,P=0.001,the differences were correlated.That is,the expression level of CEA has a correlation with the OS of CRC patients,P<0.05.The OS of CRC patients with negative CA19-9 expression(MOS 69 months)was significantly longer than that of positive patients(MOS 52 months),HR=2.949,95%CI=1.456?5.974,P=0.003,the differences were correlated.That is,CA19-9 positive test has a correlation with OS of CRC patients,P<0.05.It can be seen that the TNM stage of tumor,the degree of differentiation,and the preoperative CA19-9 and CEA expression levels are independent predictors of OS in CRC patients.Conclusion:1.The positive expressions of CEA and CA19-9 before operation cause the overall survival of CRC patients to shorten,which is inversely related to the overall survival of patients.2.The expression levels of CEA and CA19-9 before surgery,TNM stage,and tumor differentiation have similar effects in predicting the overall survival of CRC patients.
