Investigation Of The Chlamydia Trachomatis And Neisseria Gonorrhoeae Infection Status And Influencing Factors Among MSM In An Area Of Yunnan Province | | Posted on:2021-01-03 | Degree:Master | Type:Thesis | | Country:China | Candidate:L T Yu | Full Text:PDF | | GTID:2404330605481140 | Subject:Epidemiology and Health Statistics | | Abstract/Summary: | PDF Full Text Request | | Objectives:To understand the high-risk sexual behavior and the infection status of chlamydia trachomatis(CT)and gonococci gonorrhoeae(NG)among men who have sex with men(MSM)in the survey area,and to explore the influencing factors of CT and NG infection in MSM,and provide a basis for formulating relevant STD prevention and treatment strategies in this area.Methods:A cross-sectional investigation method was taken in this survey.By convenience sampling,MSM who came to Yuxi City Gay social organization to receive HIV counseling and testing services and met the inclusion criteria were recruited into this study.One-to-one questionnaire survey was conducted on them,and urine and pharyngeal swabs and rectal swabs were collected for CT and NG nucleic acid testing.The software of Epidata3.1 was used for data type-in and SPSS 17.0 was used for data statistical analysis.Results:A total of 200 respondents were recruited.One urine pharyngeal swab and one rectal swab were collected for each subject.1.Demographic characteristics:The respondents aged from 18 to 83 years old,the average age of the respondents was(38.09±14.32)years old,of which the age group of 20-39 years old accounted for 59.5%(119/200);Marital status:Most of them were unmarried,accounting for 59.0%(118/200);Degree of education:High school or technical secondary school and above accounted for 57.0%(114/200);Sex orientation:Gay accounted for 61.5%(123/200),others(heterosexual,bisexual,undetermined)accounted for 38.5%(77/200).2.Characteristics of sexual behavior:82.5%(165/200)of respondents answered that they had had anal sex with someone of the same sex in the last 6 months;5.5%(9/165)have had commercial sex with the same sex;17.0%(34/200)answered that they had had sex with the heterosexual sex in the last 6 months.The rates of consistent condom use in the last 6 months were 80.6%(133/165)and 55.9%(19/34),respectively.3.STD symptoms and history:Among the respondents,6 had experienced one or more STD related symptoms in the past 1 year;Five people have reported that they have had STD in the past year.4.Health services received:In the last year,90.5%(181/200)of MSM received peer education,and only 5.5%(11/200)of MSM received STD examination or treatment.5.CT and NG infection status:The overall prevalence of CT was 10.5%in 200 MSM investigated,the prevalence of CT in urethral,rectal and pharyngeal was 3.0%,8.0%and 0.5%,respectively,and the prevalence of rectum was the highest(P<0.05);The overall prevalence of NG was 6.5%,the prevalence of NG in urethral,rectal and pharyngeal was 0.5%,1.5%and 4.5%,respectively,and the the prevalence of pharyngeal was the highest(P<0.05).The co-infected with CT and NG rate was2.5%(5/200).6.Influence factors of CT and NG infection:NG positive(OR=5.467,95%CI:1.339-22.329)showed synergistic effect with CT infection.MSM of bath type(OR=17.722,95%CI:1.619~194.019)was a risk factor for NG infection.7.Influence factors of consistent condom use:Those with marital status of being married or living together(OR=3.986,95%CI:1.021~15.566),thoses with STD related symptoms in the last yearr(OR=16.298,95%CI:1.925~137.993)were more likely to consistently use condoms.Conclusions:The MSM in this survey was mainly young and unmarried,with a relatively high degree of education,and a wide range of occupations.Mainly through social networking software and other media to find male partners.Chlamydia trachomatis and Neisseria gonorrhoeae were more prevalent in MSM population.There was a synergistic effect between NG positive and CT infection.Those with marital status of being married or living together and thoses with STD related symptoms in the last yearr were more likely to consistently use condoms.It is suggested that in the future,we should strengthen the propaganda of STD prevention and control knowledge and the intervention of high-risk behaviors,improve the awareness of STD prevention and control and active detection of MSM.At the same time,to expand the detection coverage of CT and NG,it is necessary to conduct simultaneous CT and NG screening in the urethra,pharynx and rectum of MSM population.It is hoped that the regular screening of MSM and NG can be included in the routine STD monitoring in the future. | | Keywords/Search Tags: | MSM, Chlamydia trachomatis, Neisseria gonorrhoeae, Infection status, Influncing factors | PDF Full Text Request | Related items |
