Objectives:The screening effectiveness of three chronic obstructive pulmonary disease screening questionnaires(COPD-PS,LFQ,COPD-SQ)was verified by prospective study.Meanwhile,the screening effectiveness of three kinds of questionnaires was compared,so as to provide reference for clinical choice of COPD screening questionnaire.Methods:1?Phase?(preliminary validation of the questionnaire):a questionnaire survey was conducted among the subjects who underwent lung function tests in the outpatient pulmonary function room of the First Affiliated Hospital of Kunming Medical University from August 2019 to November 2019.According to each questionnaire,a questionnaire was issued to the screening population.The lung function tests were completed by the examinees after the completion of the questionnaires.The questionnaires were distributed,answered,recovered and arranged by the researchers themselves.2?Phase ?(comparison of the screening indexes):a questionnaire survey was carried out among the lung function patients aged 40 or above in the outpatient lung function room of the First Affiliated Hospital of Kunming Medical University from December 2019 to January 2020.Before the lung function tests were conducted,the subjects obtained three copies of the questionnaire from the researchers themselves after obtaining informed consent.After the completion of the three copies of the questionnaire,the participants completed the pulmonary function examination.The questionnaires were distributed,answered,recovered and arranged by the researchers themselves.3?Data analysis was carried out through the SPSS22.0 statistical system:A.phase ?:the scores of demographic,pulmonary function and three screening questionnaires,the total score of the questionnaire,the correlation between the cut-off value of the questionnaire and the diagnostic criteria of COPD,the correlation analysis between the total score of the questionnaire and the COPD level,the reliability and validity of the questionnaires,and the analysis of the screening evaluation indexes were compared between the COPD group and the non COPD group.B.Phase ?:three kinds of questionnaire screening evaluation indicators were compared;C.Two stage data were collected to analyze COPD risk factors.Results:1?Phase ?:The correlation coefficients between the cut-off values of each questionnaire and the COPD diagnostic criteria are:COPD-PS(r=0.555 P<0.001),LFQ(r=-0.515 P<0.001),COPD-SQ(r=0.568 P<0.001),the cut-off value of COPD-PS and COPD-SQ were positively correlated with the COPD diagnostic criteria and had a good correlation,while the cut-off value of LFQ were negatively correlated with the COPD diagnostic criteria and had a good correlation.The average total scores of COPD group in COPD-PS and COPD-SQ were larger than cut-off value,and the total scores of non COPD group were all less than cut-off value,and there was statistical difference between the total score of COPD group and non COPD group(P<0.05),the average total score of COPD group in LFQ was less than cut-off value,and the total score of non COPD group was larger than truncated value,and there was statistical difference between the total score of COPD group and non COPD group(P<0.05).The Cronbach's a of each questionnaire was COPD-PS(0.747),LFQ(0.767)and COPD-SQ(0.782)respectively.The sensitivity,specificity,false negative rate,false positive rate,positive likelihood ratio,negative likelihood ratio,Youden index and AUROC of the recommended cut-off values of each questionnaire were COPD-PS(85.19%?70.19%?14.81%?29.81%?2.86?0.21?55.38%?0.837)?LFQ(84.62%?55.75%?15.38%?44.25%?1.91?0.15?40.37%?0.747)?COPD-SQ(70.97%?72.07%?29.03%?27.93%?2.54?0.4?43.04%?0.801).2?Phase ?:Through the comparison of three kinds of questionnaire screening indicators,we found that LFQ has the highest sensitivity and AUROC,and LFQ screening efficiency is higher.3?By analyzing the multiple risk factors of the two stage COPD group and non COPD group,we found that male,high age and smoking history were independent risk factors for COPD.Low BMI may be a risk factor for COPD,and high education level may be a protective factor for COPD.Conclusions:1?COPD-PS,LFQ and COPD-SQ showed good screening performance in their respective age groups.The three screening questionnaires could be used for screening COPD.2?Comparing the screening effectiveness of three screening questionnaires among people aged 40 and above,it is found that the screening efficiency of LFQ may be more COPD-PS,LFQ and COPD-SQ.3?Men,old age and smoking history are risk factors for COPD.Low BMI may be a risk factor for COPD,and high academic qualifications may be a protective factor for COPD.
