Objective:To investigate the deep breath and effective cough exercise behavior of patients undergoing thoracoscopic pulmonary surgery,analyze and discuss its characteristics and influencing factors,and provide theoretical basis for the guidance of effective cough exercise of patients after deep breathing.Method:Based on the theory of planned behavior,through literature review,qualitative interview,expert correspondence,reliability and validity test,and by convenient sampling method,this study investigated 237 questionnaires of patients who had undergone thoracoscopic pulmonary surgery in a large-scale third-class hospital in The First Affiliated Hospital of Soochow University from August to December 2019,and included 213 questionnaires,with an effective rate of 89.87%.Using spss25.0 software statistical analysis,the main statistical methods include statistical description,independent sample t-test,one-way ANOVA,Pearson correlation analysis,exploratory factor analysis,etc.Amos 25.0 software was used to analyze the model.Result:1.The overall score of effective cough exercise after deep breathing after thoracoscopic lung surgery was 4.05±0.31,19.6%of the patients had poor exercise initiative.The average score of "active deep breathing" was 3.57±0.62,and 38.0%of the patients had poor initiative in this exercise.The average score of "taking several deep breaths" was 3.47±0.66,and 43.7%of patients had poor initiative in this exercise.The average score of "continuous cough 2-3 sound" was 3.70±0.53,and 31.5%of patients showed poor initiative in this exercise content.2.Univariate analysis showed that there were statistically significant differences in age,education level,working status and whether the patient had first pulmonary surgery(P<0.05).Pain is common in patients with a deep,effective cough.3.Behavioral attitude,subjective norm and perceived behavior control of a direct impact on behavioral intention,effect size is 0.292,0.424,0.211,respectively,behavioral intention directly affect deep breathing exercise effective cough behavior,behavioral attitude,subjective norm and perceived behavior control effectively cough through behavioral intention indirectly ACTS on the deep breathing exercise behavior,effect sizes were 0.115,0.167,and 0.261=0.174(direct effect and indirect effect=0.087).The model based on the theory of planned behavior explained 62%of the variance of behavioral intention(R2=0.62)and 38%of the variance of effective deep breathing cough exercise(R2=0.38).Conclusion:1.In patients with thoracoscopic lung surgery,the initiative of effective cough exercise after deep breathing is not ideal,and some patients have poor initiative in active deep breathing,several deep breaths and continuous coughing of 2-3 sounds.2.Patients' age and other personal factors can indirectly affect behavioral attitude,subjective norms and perceived behavioral control,and ultimately affect behavioral intention and effective cough behavior after deep breathing.The older the patient,the lower the educational level,and the worse the initiative of effective cough after deep breathing.Patients who had their first lung surgery had a deeper,more effective cough than those who did not have their first surgery.Pain has a negative effect on the patient's deep breathing and effective cough exercise.3.Subjective norms are the most important influencing factors of behavioral intention.Behavioral intention has the greatest influence on effective cough exercise behavior of deep breathing,followed by perceptual behavioral control.
